Following rectal instillation of radioiodine, liver and plasma activity-time curves were analyzed in 38 patients. Two basic hepatography patterns were found. Type I, seen in all normal controls, indicates direct rectum to liver blood flow via the portal vein. Type II represents portal-systemic collateral blood flow, and was seen in 9 of 18 patients with chronic liver disease. Of 6 patients with liver metastases, Type I was seen in 3 cases, a variant of Type I in 2 cases, and Type II in a single case. It is concluded that hepatography is useful in demonstrating portal hypertension, but of little value in the study of liver metastases.
