Gastric acid responses and plasma gastrin responses to 15-min sham feeding were determined in 4 duodenal ulcer patients with intact antrum and in 4 patients following antrum-bulb resection. Before resection a highly significant plasma gastrin response occurred during the first hour following sham feeding with a peak gastric acid response amounting to 50% of the maximal acid response to pentagastrin. After resection a non-significant increase in plasma gastrin occurred following sham feeding with a peak acid response amounting to 32% of the post-resection maximal acid response to pentagastrin. However, 1 patient showed an unequivocal post-resection gastrin response to sham feeding. The results support the concept that in duodenal ulcer patients vagal release of gastrin occurs mainly from the antrum-bulb region, and that vagally released antrum-bulb gastrin contributes to, but is not necessary for, the gastric acid response to physiological vagal activation.
