The Effect of Graded Doses of Pentagastrin and Secretin on the Intrinsic Factor(IF) Secretion in Man

By stepwise increasing doses of secretin every 30 minutes, from 0.078 to 2.1 units/kg/h, a significant reduction of volume, acid, and IF output was found, whereas the output of pepsin remained unchanged. The concentrations of acid and IF were unchanged, whereas that of pepsin increased significantly. By stepwise increasing doses of pentagastrin every 30 minutes, from 0.028 to 6.0 µg/kg/h, a continuous 2-hour secretion of IF on a high plateau level was for the first time demonstrated. An initial peak IF output was observed on each dose level. The response was similar to that of pepsin, with a lower response to 6.0 than to 1.0 µg/kg/h, in contrast to that of volume and acid. When the doses were given on different days, the responses to pentagastrin were higher after 6.0 µg/kg/h for IF and pepsin as well. Pepsin secretion continued on a high plateau level, compared to a low plateau level for IF. Possible explanations for the mode of IF secretion are discussed. The results indicate a dose-dependent secretion of IF, acid, and pepsin to 0.028 to 6.0 µg/kg/h of pentagastrin. with a different mode of secretion for the three substances.
