In brief
Most minor spleen injuries can be managed conservatively, but severe splenic ruptures often require operative treatment. In this case report, however, a young football player who severely injured his spleen was able to recover nonoperatively because he was hemodynamically stable. His case highlights issues of diagnosis, injury classification, and treatment options, as well as when to return an athlete to play.
