An analysis of the Bhopal accident

Abstract

This paper anaylzes the causes of the Bhopal accident in terms of hardware errors, operator related errors, information related errors and system related errors. The most critical reasons for the occurrence of these errors are the corporate level failure of safety management systems and procedures. If accidents are to be reduced this needs maximum attention.
