This article is a shorter version of a paper which shared joint honours in the Royal Commonwealth Society's Walter Lord Frewen essay prize competition for 1989.1 would like to acknowledge the help and advice which I received from Dr Robert Boyce of the London School of Economics, Dr Stephen Constantine of Lancaster University, Dr Dane Kennedy of the University of Nebraska and Dr Glyn Stone of Bristol Polytechnic.
