Current issues and trends related to health and population in the context of development are discussed. An underfunded health system is faced with increased disease burden in a time characterised by global shifts to market-oriented economies and reformulation of the role of the public sector. There is a need for new models for planning, organising and financing in order to be able to deal with conflicting interests and to maintain a basic commitment to equity while responding to fundamental and rapid changes in contextual realities. Selective, project-oriented donor assistance has in the past often undermined sustainability and contributed to imbalances in the health system. Norwegian assistance to the health sector is briefly discussed in relation to these issues.
