Diagnostic Arthroscopy of the TMJ: Differential Diagnoses in Patients with Limited Jaw Opening

Abstract

Arthroscopy of the temporomandibular joints was performed in patients with limited jaw movement, locking, and trismus. Three different groups of intra-articular pathological characteristics were observed. Traumatic arthritis showed features of synovitis and/or abnormal vascular network on the joint surface. Significant fibrous changes in the artcular disk, with occasional adhesive changes among the intermediate articular spaces, suggested fibrosis of the articular capsule. The last group was suspected of having anterior disk displacement; excessive stretching and occasional rupture on a part of the synovial membrane in the upper posterior pouch were frequently observed. This study indicates a higher differentiation of diagnostic value of arthroscopy for certain TM disorders.

Ken-Ichiro Murakami

Ken-Ichiro Murakami, D.D.S., D.M.Sc.

Dr. Murakami received his D.D.S. degree from Kanagawa Dental College in Yokosuka. Japan, in 1978. He advanced to the Graduate School of Medicine, Kyoto University, in 1980. and was certified in 1984. He received his D.M.Sc. degree from Kyoto University for studies on regional arthroscopic anatomy and histology of the temporomandibular joint in humans.

Dr. Murakami is assistant professor in the Department of Oral and Maxillofacial Surgery at the Kyoto University Faculty of Medicine. He has published a number of articles, book chapters, and abstracts dealing with TM disorders and with basic and clinical arthroscopic investigations on the human TMJ.

Masanori Matsuki

Masanori Matsuki, D.D.S.

Dr. Matsuki received his D.D.S. degree from Matsumoto Dental College. Japan, in 1979. He completed postgraduate training in oral and maxillofacial surgery at the Kyoto University Hospital and a general practice residency at the Kyoto City Hospital.

Dr. Matsuki belongs to a number of professional organizations. He has written articles for several professional journals and has given numerous presentations at various meetings. He is currently assistant professor of oral and maxillofacial surgery at the Kyoto University Faculty of Medicine.

Tadahiko Iizuka

Tadahiko Iizuka, D.M.D., D.M.Sc.

Dr. Iizuka is an associate professor in the Department of Oral and Maxillofacial Surgery at the Kyoto University Faculty of Medicine. He received his D.M.D. degree from Osaka Dental University. Japan. in 1965 and his D.M.Sc. degree from Kyoto University Graduate School of Medicine in 1973. He trained in oral and maxillofacial surgery under Professor Dr. T. Ono at Kyoto University Hospital and also under Professor Dr. H. L. Obwegeser at the University Clinic of Zurich. Switzerland.

Takatoki Ono

Takatoki Ono, M.D., D.D.S., D.M.Sc

Dr. Ono is a professor and chairman in the Department of Oral and Maxillofacial Surgery at the Kyoto University Faculty of Medicine. He received his M.D. and D. M. Sc. degrees from Kyoto University, and he received his D.D.S. degree from Osaka Dental College.

Dr. Ono has an instructional license from the Japanese Oral Surgeons Society, and he is a fellow of the International Association of Oral and Maxillofacial Surgeons. He is director of the Japanese Society for Jaw Deformities.
