The influence of psychodynamically oriented therapists' attachment representations on outcome and alliance in inpatient psychotherapy

Abstract

The Adult Attachment Interview (AAI) was used to study 31 psychotherapists who treated 1,381 patients in intensive multimodal inpatient psychotherapy. AAI dimensional ratings of security versus insecurity and dismissiveness versus preoccupation were used to predict alliance and outcome via multilevel regression techniques (hierarchical linear modeling). There were no main effects of therapists' attachment dimensions. However, higher attachment security of the therapist was associated with both better alliance and outcome in more severely impaired patients. Implications of the findings as well as limitations of the study are discussed.

Abstract

Unter Verwendung des Adult Attachment Interview (AAI) wurden 31 Psychotherapeuten untersucht, welche 1,381 Patienten in stationärer intensiver mulitmodaler Psychotherapie behandelten. Die dimensionalen Bewertungen des AAI in den Bereichen Sicherheit vs. Unsicherheit und Distanziertheit (Dismissiveness) vs. Verstricktheit (Preoccupation), wurden verwendet um mit Hilfe von Mehrebenen-Regressions-Analysen (Hierarchical Linear Modeling) therapeutische Allianz und Therapieergebnis vorherzusagen. Es ergaben sich keine Haupteffekte für die Bindungsdimensionen des Therapeuten. Allerdings stand eine stärkere Bindungssicherheit des Therapeuten, sowohl mit einer besseren Allianz, wie auch mit einem besseren Therapieergebnis bei Patienten mit schweren Beeinträchtigungen im Zusammenhang. Schlussfolgerungen bzgl. der Ergebnisse und Einschränkungen der Studie werden diskutiert.

L'entretien d'attachement adulte (AAI) a été utilisé pour étudier 31 psychothérapeutes ayant traités 1'381patients en psychothérapie hospitalière intensive multimodale. Les dimensions d'attachement de sécurité versus insécurité et dédain versus préoccupation ont été utilisées pour prédire l'alliance et les résultats à l'aide de techniques de régression multi-niveaux (HLM). Il n'y avait pas d'effet des dimensions d'attachement du thérapeute. Cependant, une sécurité plus importante du thérapeute était associée avec à la fois une meilleure alliance et de meilleurs résultats chez les patients les plus sévèrement détériorés. Les implications de ces résultats ainsi que les limitations de l’étude sont discutées.

A Adult Attachment Interview (AAI) foi utilizada para estudar 31 psicoterapeutas que trataram 1.381 pacientes em psicoterapia intensiva multimodal em regime de internamento. As avaliaç[otilde]es dimensionais de segurança da AAI versus insegurança e desligadamento versos preocupação foram utilizadas para predizer a aliança e os resultados terapêuticos através de técnicas de regressão (modelos lineares hierárquicos). Não se encontraram efeitos do terapeuta nas dimens[otilde]es de vinculação. No entanto, maior segurança na vinculação do terapeuta estava associada tanto com melhores alianças como resultados terapêuticos em pacientes com maior nível de disfuncionalidade. As implicaç[otilde]es dos resultados, bem como as suas limitaç[otilde]es são discutidos.

L'Adult Attachment Interview (AAI) è stato utilizzato per studiare 31 psicoterapeuti che hanno trattato 1.381 pazienti in una terapia multimodale ospedaliera intensiva. Sono state utilizzate dell'AAI le dimensioni della sicurezza versus l'insicurezza e della spensieratezza versus la preoccupazione sono stati utilizzati per predire l'alleanza e il risultato attraverso tecniche di regressione multilivello (modellazione gerarchica lineare). Non sono stati ottenuti particolari effetti significativi sull'attaccamento dei terapeuti. Tuttavia, l'attaccamento sicuro del terapeuta è stato associato ad una alleanza migliore e a risultati buoni in pazienti pi[ugrave] gravemente compromessi. Le conseguenze di tali risultati, nonché le limitazioni dello studio sono discussi di seguito.
