The therapist's attachment representation and the patient's attachment to the therapist

Abstract

This study examines the relationship between the therapist's own attachment representation (Adult Attachment Interview, AAI) and the patient's attachment relationship to the therapist (client's attachment to the therapist, CATS). The attachment representations of n=22 psychotherapists who treated n=429 patients were assessed. A general effect of the secure/insecure therapist attachment status on the attachment of the patient to the therapist was not found whereas the more specific effects expected could be confirmed: The more preoccupied the therapist's attachment status was, the more the patient experienced a preoccupied-merger attachment to the therapist. The more dismissing the therapist's attachment status was, the more the patient experienced an avoidant-fearful attachment to the therapist. In summary, not the general security/insecurity of the therapist's attachment representation but rather the type of the insecurity is associated with the subjective patient's attachment-related experience of the therapeutic dyad.

Die Studie untersuchte den Zusammenhang zwischen der Darstellung der Bindungsrepräsentation des Therapeuten (Adult Attachment Interview, AAI) und der Bindungsbeziehung des Patienten zu seinem Therapeuten (Client's attachment to the therapist, CATS). Die Darstellung der Beziehungsrepräsentationen wurden von 22 Psychotherapeuten welche 429 Patienten behandelten erhoben. Ein allgemeiner Effekt des sicher/unsicheren Bindungsstatus des Therapeuten auf die Bindung der Patienten zu den Therapeuten wurde nicht gefunden, während die erwarteten spezifischen Effekte bestätigt werden konnten: Je voreingenommener der Bindungsstatus des Therapeuten war, desto eher erlebten die Patienten eine voreingenommen-vermischte Beziehung zu ihren Therapeuten. Je ablehnender der Beziehungsstatus des Patienten war, desto eher erlebten die Patienten eine vermeidend-ängstlich Beziehung zu ihren Therapeuten.

Zusammenfassend stehen nicht die generellen sicheren/unsicheren Bindungsrepräsentationen der Therapeuten, sondern die Art der Unsicherheit in Zusammenhang mit der subjektiven Bindungserfahrung der Patienten innerhalb der Therapiedyade

Questo studio esamina la relazione tra la rappresentazione di attaccamento del terapeuta (Adult Attachment Interview, AAI) e la relazione di attaccamento del paziente al terapeuta (attaccamento del cliente al terapeuta, CATS). Sono state valutate le rappresentazioni di attaccamento di n = 22 psicoterapeuti che hanno curato n = 429 pazienti. Non è stato rilevato alcun effetto generale dello stato di attaccamento sicuro/insicuro del terapeuta sull'attaccamento del paziente al terapeuta mentre gli effetti più specifici previsti sono stati confermati: più lo stato di attaccamento del terapeuta era preoccupato, più il paziente sperimentava un attaccamento al terapeuta di tipo preoccupato-invischiato. Quanto più lo stato di attaccamento del terapeuta era evitante, tanto più il paziente sperimentava un attaccamento al terapeuta evitante-spaventato. In sintesi, non è la generale sicurezza/insicurezza della rappresentazione dell'attaccamento del terapeuta, ma piuttosto il tipo di insicurezza ad essere associato all'attaccamento personale del paziente in relazione all'esperienza della diade terapeutica

Este estudo avaliou a representação da vinculação do terapeuta (Adult Attachment Interview, AAI) e a relação de vinculação do paciente com o terapeuta (vinculação do cliente com o terapeuta, CATS). As representações de vinculação foram avaliadas em (n_22) psicoterapeutas que trataram (n_429) pacientes. Não foi encontrado um efeito geral do tipo de vinculação seguro/inseguro do terapeuta sobre o tipo de vinculação do paciente com o terapeuta, considerando que os efeitos mais específicos esperados pudessem ser confirmados. A maior preocupação do tipo de vinculação do terapeuta foi a quantidade de fusão de preocupações na vinculação do terapeuta experienciada pelo paciente. O maior afastamento do tipo de vinculação do terapeuta foi o paciente ter experienciado vinculação de afastamento-medo com o terapeuta. Em suma, não a representação da vinculação do terapeuta de segurança/insegurança em geral, mas sim o tipo de insegurança que está associado à experiência subjectiva da vinculação-relacionada do paciente da díade terapêutica.

Este estudio evalúa la relación entre el tipo de representación de apego de los terapeutas, analizada a través de la Entrevista de Apego Adulto (AAI), y el tipo de apego que establecen los pacientes en relación a sus terapeutas (apego de los pacientes a los terapeutas CATS). Se evaluaron las representaciones de apego de n = 22 terapeutas que trataron n = 429 pacientes. No se halló que hubiera un efecto general del tipo de apego seguro/inseguro del terapeuta sobre el tipo de apego de los pacientes mientras que, por otro lado, pudieron confirmarse los efectos esperados de los tipos más específicos de apego que se esperaba hallar. Cuanto más preocupado era el estilo de apego del terapeuta, más el paciente mostraba un estilo preocupado-fusional. Cuando más rechazante era el estilo del terapeuta, más temeroso-evitativo era el estilo del paciente. En suma, no es tan relevante el hecho que el terapeuta tenga un tipo de representaciones de apego seguro o inseguro, sino el tipo de apego inseguro que aparezca en las representaciones de apego del terapeuta lo que influye en el tipo de experiencia subjetiva de apego que el paciente establece con él.
