“Just like talking to someone about like shit in your life and stuff, and they help you”: Hopes and expectations for therapy among depressed adolescents

Abstract

Objective: To explore hopes and expectations for therapy among a clinical population of depressed adolescents. Method: As part of a randomized clinical trial, 77 adolescents aged 11–17, with moderate to severe depression, were interviewed using a semi-structured interview schedule. The interviews were analysed qualitatively using framework analysis. Results: The findings are reported around five themes: “the difficulty of imagining what will happen in therapy,” “the ‘talking cure,’” “the therapist as doctor,” “therapy as a relationship,” and “regaining the old self or developing new capacities.” Conclusions: Differing expectations are likely to have implications for the way young people engage with treatment, and failure to identify these expectations may lead to a risk of treatment breakdown.

Obiettivo: esplorare le speranze e le aspettative sulla terapia in una popolazione clinica di adolescenti depressi. Metodo: nell'ambito di uno studio clinico randomizzato, sono stati intervistati 77 adolescenti di età compresa tra 11 e 17 anni, con depressione da moderata a grave, utilizzando un programma di intervista semi-strutturata. Le interviste sono stati analizzate qualitativamente utilizzando l'analisi della struttura. Risultati: I risultati sono riportati intorno a cinque temi: "la difficoltà di immaginare ciò che accadrà nella terapia", "la cura del parlare'", "il terapeuta come medico", "terapia come una relazione," e "riappropriarsi del vecchio sé o sviluppare nuove capacità ". Conclusioni: differenti aspettative possono avere implicazioni nel modo in cui i giovani si impegnano nel trattamento, e la mancata individuazione di queste aspettative può portare ad un rischio di interruzione del trattamento.

Método: Como parte de uma amostra clínica aleatória, 77 adolescentes com idades compreendidas entre os 11 e os 17 anos, com depressão moderada a severa, foram entrevistados utilizando um guião de entrevista semiestruturado. As entrevistas foram analisadas qualitativamente utilizando uma análise estrutural. Resultados: Os resultados reportam-se em torno de cinco temas: “a dificuldade em imaginar o que acontecerá na terapia”, “a cura pela palavra”, “o terapeuta enquanto médico”, “a terapia enquanto relação”, e “recuperar o velho eu ou desenvolver novas capacidades”. Conclusões: Diferenciar expectativas tende a ter implicações no modo como os jovens se envolvem no tratamento, e fracassar na identificação dessas expectativas pode colocar o tratamento em causa.

Zusammenfassung Ziel: Untersuchung von Hoffnungen und Erwartungen an die Therapie in einer klinischen Population depressiver Jugendlicher. Methode: Als Teil einer randomisierten klinischen Studie wurden 77 Jugendliche im Alter von 11-17 Jahren mit einer mittelgradigen bis schweren Depression anhand eines semi-strukturierten Interviews befragt. Die Interviews wurden mit einer Framework Analyse qualitativ ausgewertet. Ergebnisse: Die Ergebnisse wurden im Rahmen von fünf Themen berichtet: ”Schwierigkeit mit der Vorstellung von dem, was in einer Therapie passiert“, ”die Heilung durch das Sprechen“, ”der Therapeut als Arzt“, ”Therapie als eine Beziehung“ und ”das alte Selbst wiedergewinnen oder neue Fähigkeiten entwickeln.“ Schlussfolgerung: Unterschiedliche Erwartungen haben wahrscheinlich Implikationen für die Art, wie sich junge Menschen in die Behandlung einbringen. Das Versäumnis diese Erwartungen zu erkennen, kann zu einem Risiko für Behandlungsabbrüche führen.
