Effects of providing domain specific progress monitoring and feedback to therapists and patients on outcome

Abstract

Objective: Progress monitoring and feedback reduces the number of patients deteriorating in psychotherapy. The current study examined the effects of providing treatment progress information to therapists and patients using individual feedback of both wellbeing and affective psychological distress compared to feedback of wellbeing alone. Method: The sample comprised 845 consecutive psychiatric day-hospital admissions using a historical cohort design. The effects of monitoring and feedback of wellbeing in Cohort 1 were compared against the effects of monitoring and feedback of both wellbeing and affective psychological distress in Cohort 2. Results: Patients who were “not-on-track” in Cohort 2 demonstrated significantly greater improvement for affective psychological distress than those from Cohort 1. Conclusions: These findings suggest that providing feedback from multiple sources enhances patient outcomes in comparison to single source feedback.

Ziel: Monitoring- und Feedbackverfahren reduzieren die Anzahl der Patienten, dessen Zustand sich während der Psychotherapie verschlechtert . Diese Studie untersuchte, welchen Effekt die Bereitstellung von Informationen zum Behandlungsfortschritt für Therapeuten und Patienten hat. Hierzu wurde die individuelle Rückmeldung von Wohlbefinden und affektivem psychologischen Leiden mit der Rückmeldung von ausschließlich Wohlbefinden verglichen. Methode: Die Stichprobe umfasst 845 aufeinanderfolgende Aufnahmen in eine psychiatrische Tagesklinik. Es wurde ein retrospektives Kohortendesign genutzt. Die Effekte von Monitoring und Feedback von Wohlbefinden in Kohorte 1 wurden mit den Effekten von Monitoring und Feedback von Wohlbefinden und affektivem psychologischen Leiden in Kohorte 2 verglichen. Ergebnisse: Patienten, die in Kohorte 2 nicht den erwarteten Fortschritt machten, zeigten eine signifikant stärkere Verbesserung des affektiven psychologischen Leidens als solche Patienten in Kohorte 1. Questo studio ha esaminato gli effetti del fornire informazioni sull'andamento del trattamento a terapeuti e pazienti attraverso feedback individuali sia sul benessere che sul disagio psicologico affettivo oppure fornendo feedback solo del benessere. Metodo: Il campione comprendeva 845 ricoveri consecutivi in day-hospital psichiatrico in un disegno di ricerca di coorte storico. Sono stati confrontati gli effetti del monitoraggio e del feedback di benessere nella Coorte 1 con gli effetti del monitoraggio e del feedback sia del benessere che del disagio psicologico affettivo nella Coorte 2. Risultati: I pazienti che erano "non in pista" nella Coorte 2 hanno mostrato un miglioramento significativamente superiore nel disagio psicologico affettivo rispetto a quelli della Coorte 1. O presente estudo examinou os efeitos do fornecimento de informação acerca do progresso do tratamento aos terapeutas e pacientes, dando feedback individual acerca quer do bem-estar quer do sofrimento psicológico afetivo simultaneamente, em comparação com o fornecimento de feedback acerca do bem-estar apenas. Método: A amostra incluiu 845 admissões consecutivas num hospital psiquiátrico de dia, utilizando um desenho de corte histórico. Os efeitos da monitorização e do feedback acerca do bem-estar do Grupo de Corte 1 foram comparados com os efeitos da monitorização e do feedback acerca do bem-estar e do sofrimento psicológico afetivo em simultâneo no Grupo de Corte 2. Resultados: Os pacientes que estavam “não no caminho certo” no Grupo de Corte 2 demonstraram uma melhoria significativamente superior relativamente ao sofrimento psicológico afetivo do que os do Grupo de Corte 1. Conclusões: Estes resultados sugerem que o fornecimento de feedback com base em múltiplas fontes potencia os resultados dos pacientes em comparação com o feedback realizado a partir de uma só fonte.

1 For the current analysis only subscale scores for the Outcome Measures were made available making calculation of Cronbach's α for the current sample impossible. There is no reason to suspect that the internal consistency of these measures would vary from those reported.

2 Group therapy patients who do not return to a nonclinical population by the end of therapy may choose to continue individual outpatient therapy.

3 In Wellbeing Alone Feedback therapists were able to view their patients WHO-5 graphs at any point during therapy through touchscreens present in the treatment rooms.

4 In Wellbeing and Distress Feedback therapists were able to view their patients WHO-5 and DI-5 graphs at any point during therapy through touchscreens present in the treatment rooms.

5 A morning session of Day 5 is set aside for a thorough discussion of feedback with printed graphs. The expected response to treatment trajectories are available at any point in time to the therapists via computer and there was no restriction as to when therapists could review these charts.
