25 Years of systemic therapies research: Progress and promise

Abstract

Objective: In this article we describe and assess the state of the science on systemic psychotherapies. In the quarter century since the first issue of Psychotherapy Research was published, considerable progress has been made. There is an increasingly solid evidence base for systemic treatments, which includes a wide range of approaches to working conjointly with couples and families. Moreover, there are exciting new developments that hold promise for explicating the dynamic processes of therapeutic change in couple and family systems. Method: We begin by explaining how we view “systemic therapies” as different from individual approaches and then summarize what we have learned in the past 25 years about this set of treatments, how we have learned it, and what we have yet to learn. Results and Conclusions: We consider current trends in research on outcomes and change process mechanisms, and end with speculations about what lies ahead in the interrelated domains of systemic research and practice.

Obiettivo: In questo articolo descriviamo e valutiamo lo stato dell'arte nelle psicoterapie sistemiche. Nei 25 anni seguenti alla pubblicazione del primo numero di Psychotherapy Research, sono stati compiuti notevoli progressi. Vi sono sempre più solide evidenze alla base dei trattamenti sistemici, che includono una vasta gamma di approcci nel lavoro congiunto con coppie e famiglie. Inoltre, ci sono nuovi interessanti sviluppi che mantengono la promessa di spiegare i processi dinamici del cambiamento terapeutico nella coppia e nei sistemi familiari. Metodo: all’inizio viene descritto come le "terapie sistemiche" sono diverse rispetto agli approcci individuali; quindi viene riassunto tutto ciò che è stato appreso negli ultimi 25 anni su questi trattamenti, come lo abbiamo imparato, e cosa dobbiamo ancora imparare. Risultati e conclusioni: Saranno valutate le attuali tendenze della ricerca sugli esiti e sui meccanismi di cambiamento di processo, e infine saranno presentate delle riflessioni su ciò che ci aspetta nei settori correlati di ricerca sistemica e pratica.

目的：本文描述並評估系統治療的狀態。從心理治療研究第一期發行至今25年來，已有顯著的進展。關於運用各種不同取向與伴侶或家庭工作的系統治療的實證結果日益增多。而且，解釋伴侶及家庭系統的治療改變歷程的論述，也有令人振奮的發展。方法：先說明如何區別系統治療與個別治療取向的不同，接著摘要過去25年來，在系統治療中的所學、如何學，與尚待學習的。結果與結論：考量當代治療結果及改變過程機制的研究趨勢，推測系統治療研究與實務領域未來結合的可行方向。

Zusammenfassung:

Ziel: In diesem Artikel beschreiben und beurteilen wir den Stand der Wissenschaft zur systemischen Psychotherapie. Seit der Veröffentlichung der ersten Ausgabe von Psychotherapy Research im letzten Vierteljahrhundert hat ein beachtlicher Fortschritt stattgefunden. Es gibt eine wachsende solide Evidenz für die systemische Behandlung, die ein weites Spektrum von Ansätzen für gemeinsame Arbeit mit Paaren und Familien beinhaltet. Des Weiteren gibt es spannende neue Entwicklungen, die eine Erklärung von dynamischen therapeutischen Veränderungsprozessen in Paar- und Familiensystemen bieten. Methode: Zu Beginn beschreiben wir unsere Sicht auf die systemische Therapie in Kontrast zu anderen individuellen Ansätzen und fassen anschließend zusammen, was wir in den letzten 25 Jahren über diese Therapierichtung gelernt haben, wie wir dies gelernt haben und was wir noch lernen müssen. Ergebnisse und Schlussfolgerung: Wir betrachten aktuelle Trends der Ergebnis- und Prozessforschung, und schließen mit Vermutungen zur zukünftigen Forschung und Praxis der systemischen Therapie ab.

Objetivo: Neste artigo descrevemos e avaliámos o estado-de-arte das psicoterapias sistémicas. Ao longo do quarto de século desde que a primeira edição da Psychotherapy Research foi publicada, realizou-se um progresso considerável. Existe uma base de evidência cada vez mais sólida para tratamentos sistémicos, os quais incluem uma ampla gama de abordagens para trabalhar conjuntamente com casais e famílias. Além disso, novos desenvolvimentos de interesse prometem explicar os processos dinâmicos da mudança terapêutica em sistemas de casal e familiares. Método: Iniciamos explicando por que consideramos distintas as “terapias sistémicas” e as abordagens individuais; resumindo, depois, o que aprendemos nos últimos 25 anos acerca deste conjunto de tratamentos, como o aprendemos, bem como o que ainda falta aprender. Resultados e Conclusão: Consideramos as tendências atuais na investigação sobre os resultados e os mecanismos processuais de mudança e terminamos com especulações sobre o que está por vir nos domínios interrelacionados de investigação e prática sistémica.
