Patient-focused and feedback research in psychotherapy: Where are we and where do we want to go?

Abstract

In the last 15 years feedback interventions have had a significant impact on the field of psychotherapy research and have demonstrated their potential to enhance treatment outcomes, especially for patients with an increased risk of treatment failure. This article serves as an introduction to the special issue on “Patient-focused and feedback research in psychotherapy: Where are we and where do we want to go?” Current investigations on feedback research are concerned with potential moderators and mediators of these effects, as well as the design and the implementation of feedback into routine care. This introduction summarizes the current state of feedback research and provides an overview of the three main research topics in this issue: (1) How to implement feedback systems into routine practice and how do therapist and patient attitudes influence its effects?, (2) How to design feedback reports and decision support tools?, and (3) What are the reasons for patients to become at risk of treatment failure and how should therapists intervene with these patients? We believe that the studies included in this special issue reflect the current state of feedback research and provide promising pathways for future endeavors that will enhance our understanding of feedback effects.

Negli ultimi 15 anni gli interventi di feedback hanno avuto un impatto significativo nel campo della ricerca in psicoterapia e hanno dimostrato il loro potenziale nel migliorare gli esiti del trattamento, specialmente per i pazienti con un rischio maggiore di fallimento del trattamento. Questo articolo è l'introduzione al numero speciale su "Ricerca focalizzata sul paziente e sul feedback in psicoterapia: Dove siamo e dove vogliamo andare?". Le indagini in corso sulla ricerca di feedback riguardano i potenziali moderatori e i mediatori di questi effetti, nonché la progettazione e lo sviluppo del feedback nella pratica di routine. Questa introduzione riassume lo stato attuale della ricerca di feedback e fornisce una panoramica dei tre principali temi di ricerca in questo numero: (1) Come implementare sistemi di feedback nella pratica di routine e come gli atteggiamenti del terapeuta e del paziente influenzano i suoi effetti ?, (2) Come progettare report di feedback e strumenti di supporto alle decisioni?, e (3) Quali sono i motivi per i pazienti di diventare a rischio di fallimento del trattamento e come i terapeuti dovrebbero intervenire con questi pazienti? Noi crediamo che gli studi inclusi in questo numero speciale riflettano lo stato attuale della ricerca di feedback e indichino percorsi promettenti per gli sforzi futuri che miglioreranno la nostra comprensione sugli effetti del feedback.

Zusammenfassung: In den letzten fünfzehn Jahren hatten Feedbackinterventionen einen signifikanten Einfluss auf die Psychotherapieforschung und haben ihr Potential zur Verbesserung von Behandlungsergebnissen demonstriert, besonders bei Patienten mit einem erhöhten Risiko für eine erfolglose Behandlung Dieser Artikel dient als eine Einführung zur Sonderausgabe zum Thema „Patientenorientierte und Feedbackforschung in der Psychotherapie: Wo sind wir und wo wollen wir hin?“. Aktuelle Untersuchungen zur Feedbackforschung beschäftigen sich mit potentiellen Moderatoren und Mediatoren dieser Effekte, sowie mit dem Design und der Implementierung des Feedbacks in die Routineversorgung. Diese Einführung fasst den aktuellen Stand der Feedbackforschung zusammen und liefert einen Überblick über die drei Hauptforschungsthemen in dieser Ausgabe: (1) Wie können Feedbacksysteme in die laufende Praxis implementiert werden und wie beeinflussen die Einstellungen der Therapeuten und Patienten diese Effekte? (2) Wie können Feedbackberichte und Entscheidungshilfen gestaltet werden? und (3) Was sind die Gründe dafür, dass Patienten ein erhöhtes Risiko für eine erfolglose Behandlung entwickeln und wie sollten Therapeuten bei diesen Patienten intervenieren? Wir glauben, dass die Studien, die in dieser Sonderausgabe enthalten sind, den aktuellen Stand der Feedbackforschung reflektieren und vielversprechende Wege für zukünftige Forschungsbemühungen aufzeigen, die unser Verständnis für Feedbackeffekte verbessern werden.

Nos últimos 15 anos, as intervenções de feedback tiveram um impacto significativo na investigação em psicoterapia e demonstraram o seu potencial no melhoramento dos resultados do tratamento, especialmente para pacientes com um risco acrescido de fracasso. Este artigo serve como uma introdução ao número especial acerca da “Investigação em psicoterapia focada no paciente e no feedback: Onde estamos e para onde queremos ir?”. Os estudos atuais no âmbito da investigação acerca do feedback estão preocupados com potenciais moderadores e mediadores destes efeitos, assim como com o desenho e a implementação do feedback nos serviços de saúde. Esta introdução sumaria o estado atual da investigação acerca do feedback e fornece uma visão geral dos três tópicos de pesquisa neste número: 1) Como implementar sistemas de feedback nas práticas de saúde e como é que as atitudes do terapeuta e do paciente influenciam estes efeitos?; 2) Como desenhar registos de feedback e ferramentas de apoio à decisão clínica?; e, 3) Quais as razões para os pacientes se encontrarem em risco de fracasso no tratamento e como devem os terapeutas intervir com esses pacientes?. Acreditamos que os estudos incluídos neste número especial refletem o estado atual da investigação acerca do feedback e fornecem trajetórias promissoras para empreendimentos futuros que aumentarão a nossa compreensão sobre os efeitos do feedback.

過去十五年心理治療研究領域已經證實透過病人回饋做為治療介入的方式，具有提升治療的潛在效果，特別是那些快要治療失敗的病人。這篇文章特別介紹這個特別議題—心理治療中”以病人為中心”的回饋研究，“我們現在在那裡?又我們想往那裡去?”，當前回饋研究取向關注影響治療效果的潛在調節變項、中介變項以及如何在研究設計、定期照護中執行病人的回饋機制。這篇文章主要在介紹、總結回饋研究當前的發展狀況，並提出三大研究主題：(1) 如何在定期的心理治療實務中實施回饋系統，治療師和病人的態度又如何影響治療效果?(2)如何構思回饋研究的報告?又如何決定輔助工具的使用?(3)還有那些是治療失敗的危機因素，治療師該如何針對這些病人採取介入措施?我們相信包括上述這些特別的議題能夠反應當今回饋研究的現況，並提供一些未來值得努力的研究方向，有助於提升我們對回饋研究成果的了解。
