Central elements in relapse prevention procedures with sex offenders

Abstract

This paper reviews the major elements of the relapse prevention model as it has been applied to sex offenders. The historical development of treatment for sex offenders is described, emphasizing the pivotal role of behaviour therapy and the eventual development of cognitive-behaviour therapy. Relapse prevention as originally conceived is then compared to its application to sex offenders. A cognitive-behavioural model of relapse is presented, followed by descriptions of the central elements of that model. Specifically, these are the cognitive-behavioural chain, the sexual abuse cycle, seemingly irrelevant decisions, high-risk situations and risk factors, offence precursors, coping responses, and the use of reminder cards. It is concluded that, while relapse prevention programs have proven quite popular, there is as yet insufficient evidence for their efficacy compared to more traditional cognitive-behavioural interventions. Continuing testing of the model with sex offenders is encouraged.
