Permanent t-tube insertion in two patients with Hurler's syndrome

Abstract

Hearing loss in mucopolysaccharidosis is usually both conductive and sensorineural. The conductive component is attributable to serous otitis media secondary to dysfunction of the eustachian tube and chronic thickening of the mucosa of the middle ear. The conductive component may persist after myringotomy and insertion of short-term or long-term ventilation tubes. In Hurler's syndrome, death usually occurs in the first decade of life. In our study, we present two cases, a three-year-old girl and a four-year-old boy, who were diagnosed with Hurler's syndrome. Both children have a history of otitis media with effusion requiring repeated short-term ventilation tube insertions that were unsuccessful. Permanent t-tubes were inserted in both cases. Results showed an approximate 20 dB improvement in hearing sensitivity postoperatively for each patient.

Sumario

La alteración auditiva en las mucopolisacaridosis es tanto conductiva como sensorineural. El componente conductivo se atribuye a la otitis media serosa secundaria a la disfunción de la trompa de Eustaquio y al engrosamiento crónico de la mucosa del oído medio. El componente conductivo puede persistir luego de una miringotomía y de la inserción de tubos de ventilación a corto o a largo plazo. En el síndrome de Hurler la muerte suele ocurrir en la primera década de la vida. En nuestro estudio, presentamos dos casos – una niña de tres años y un niño de cuatro años – con diagnóstico de síndrome de Hurler. Ambos niños tenían una historia de otitis media con efusión que requirió la inserción repetida de tubos de ventilación transitoria, con resultados no exitosos. Se insertaron tubos permanentes en T en ambos casos. Los resultados mostraron una mejoría de aproximadamente 20 dB en la sensibilidad auditiva post-operatoria en cada paciente.
