Abstract
ChristianaCare developed a strategic plan to address the health literacy needs of patients, families and the community served. This description is an example model of building a case for health literacy within a health system and planning for the long-term sustainment of organizational change including advocacy with executive leadership, infrastructure adjustments to improve consistency of content and resource commitment. Key structural and cultural changes within the system aim to secure a lasting framework, meeting the health literacy challenges facing all participants in the continuum of care.
