Cancer Primitif Du Foie Et HypoglycÉmie

Summary

We report a case of a primary carcinoma of the liver in a 27 years old male, where the injection of HI units Insulin resulted in a deep coma with hypoglycemia of 14 ctgr %c.

Twenty four and fourty eight hours after the first hypoglycemic incident, the fasting blood-sugar has been found respectively 29 and 47 ctgr%c.

Liver post-mortem examination : cirrhosis, hepatoma.

This observation, together with similar cases reported in the literature, shows that liver hepatoma can be accompanied by hypoglycemia without any clinical manifestation.

The attention is drown upon the great insulin sensitivity in these patients, even with very small doses of insulin.

Nous déerivons un cas de cancer primitif du foie chez un homme do 27 ans, chez qui uno injection de 10 unites dinsuline ordinaire a provoqué un coma profond avec une glycémie de 14 ctgrs %c.

La glycémie à jeun, détermince 24 et 48 heures après cet incident a été trouvée respectivement de 29 et 47 ctgrs %t.

Exitus dans le coma 5 jours après l’accident hypoglycémique.

L’examen anatomo-pathologique a révélé un foie de 5,2 Kgrs cirrliotique -et siege d’un hépatome.

Cette observation, jointe à d’autres similaires de la littérature montre que l’hypoglycémie spontanée, accompagnant parfois la néoplasie hépatique, no se inanifeste pas nécessairement par des signes cliniques.

L’attention est attirée sur la grande sensibilité de ces malades vis-à-vis des doses, méme minimes, d’insuline.

Bij een man van 27 jaar oud word een primair levercarcinoom gevonden. Tien éénheden gewone insuline venvekken een diep coma met glyeiemie dalend tot 14 centgr %c.

Nuchtere glyeiemie, 21 on 48 uur na dit incident was respectievelijk 29 en 47 centgr %c.

Exitus volgde in coma, vijf dagen na de bypoglyoiemie. Sectie werd gedaan.

De lever woog 5,2 Kgr, was cirrliotisc met een hepatoom.

Dit geval, evenals andere in de literatuur besclireven, bewijst dat de spontane bypoglyoiemie, die leverneoplasma soms begeleidt, niet noodzakelijk kliniscbe syinptomen dient te geven.

Aandacht wordt getrokken op bet feit, dat doze zieken zeer gevoelig zijn zelfs nan kleine dosissen insuline.
