Summary

The authors report a new case of Thrombotic thrombocytopenic purpura (rapid fatal evolution in a 47 years old female, although treated with prednisone; haematological findings, including serological studies that gave negative results; post-mortem examination confirming the diagnosis). The authors review the pathogenic discussion (on pathological and physio-pathological grounds) about this syndrome, which might be classified, besides collagen diseases and periarteritis nodosa, amongst pathological reactions to hypersensibilization.

Les auteurs décrivent un nouveau cas de purpura thrombocytopénique thrombotique (observation clinique : évolution rapidement fatale chez une femme de 47 ans, traitée par la prednisone; examens de laboratoire comprenant la recherche d’anticorps, qui fut négative; autopsie confirmant le diagnostic posé ante mortem). Ils exposent les discussions pathogéniques que soulève ce syndrome (aux points de vue anatomo-pathologique et physio-pathologique), qu’on peut proposer de classer actuellement, au voisinage des maladies du collagène et de la périartérite noueuse, dans le groupe des manifestations pathologiques d’hypersensibilisation.

Beschrijving van een geval van thrombotisch thrombocytopenisch purpura (klinische evolutie van het geval : snel dodelijke afloop bij een 47 jaar oude vrouw, ondanks behandeling door prednisone; laboratorium onderzoekingen, met poging tot het vaststellen van aanwezigheid van anticorpsen (Rbc of plaquetten) met negatieve uitslag; post-mortem onderzoek dat de diagnose bevestigt). Op anatomo-pathologisch en physio-pathologisch gebied wordt geredetwist over de pathogenie van dit syndroom, dat men, benevens collageen ziekten en periarteritis nodosa, in de groep van de pathologische reacties van hypersensibilisatie zou kunnen rangschikken.
