Osteomalacie Par Pyelonephrite

Summary

A case showing clinical and radiological evidence of osteomalacia associated with pyelonephritis is reported. Right nephrectomy was performed because there was pyonephrosis on this side.

Surgical procedure was followed by complete cure of the osteopathy. Metabolic data (table VII) indicate hypophosphatemia, hypomagnesiemia and excessive urinary loss of P, Mg and Ca before operation.

Although a slight systemic acidosis was present, no abnormality of renal acid producing mechanisms was elicited.

After operation renal handling of P, Ca and Mg was shown to be essentially normal.

These facts demonstrate that, in this case, the renal tubular defect responsible for metabolic disturbances was limited to the right kidney.

Notre observation décrit un cas d’ostéomalacie due à une pyélonéphrite chronique. Une exploration métabolique n’a pas mis en évidence une impossibilité du rein à sécréter de l’ammoniaque ou à éliminer des ions H⁺, malgré une légère acidose sanguine. Par contre, il existait une fuite rénale de phosphore, de calcium et de magnésium. Le tableau VII du texte résume les constatations biologiques pré- et post-opératoires. L’ablation du rein droit atteint de pyonéphrose a corrigé totalement le désordre humoral. L’état clinique s’est amélioré considérablement et l’examen radiologique a montré une régression spectaculaire du syndrome de Milkman. Ces faits démontrent que l’altération rénale responsable de l’ostéomalacie était localisée à l’un des deux reins.

Wij beschrijven een geval van osteo-malacie te wijten aan chronische pyelonephritis. Niettegenstaande een lichte plasmatische acidose bleek de nier in staat ammoniak en waterstofionen uit te scheiden. Er bestond daarentegen een renaal verlies van phosphor, calcium en magnesium. Tabel VII van de tekst resumeert de biologische pre- en post-operatoire bevindingen. Wegname van de rechter nier, aangetast door pyo-nephrose, normaliseerde de humorale afwijkingen. Het klinisch beekl verbeterde in belangrijke mate en het radiologisch onderzoek toonde een spectaculaire regressie van het Milkman syndroom. Deze feiten bewijzen dat de renale afwijkingen die verantwoordelijk waren voor de osteo-malacie in één nier gelocaliseerd waren.
