Beschouwingen Over De Ziekte Van Kahler Aan De Hand Van Een Vroecsymptoom : Recidiverende Longinfecties

Summary

In 8 cases of multiple myelomatosis, 5 presented pulmonary infections. Three times, this pulmonary complication was the revelating symptom of multiple myeloma, two times a secondary one.

Observing the frequency of recidivating pneumococcal infection in this disease, the author discusses poor antibody response in Kahler’s disease, and emits some hypotheses about paraprotein structure.

Comparison is made between plasmocellular anomalies in myelomatosis and agammaglobulinemia, and based thereupon the hypothesis of a common origin for both affections is proposed : reticulosis in the largest sense of the word.

Dans 8 cas de maladie de Kahler, l’auteur a constaté 5 fois des infections pulmonaires. Trois fois il s’agissait d’une « pneumopathie révélatrice », deux fois d’une pneumopathie secondaire. Partant de la fréquence des récidives d’infections pneumococciques chez ces malades, l’auteur engage une discussion du problème de la formation défectueuse d’anticorps dans la maladie de Kahler et formule quelques hypothèses à propos de la structure des paraprotéines.

Se basant sur les anomalies plasmocytaires dans la myélomatose et l’agammaglobulinémle, l’auteur émet l’hypothèse d’une parenté pathogénique entre ces deux affections qui peuvent être reliées à une source commune : la réticulose au sens le plus large.

Samenvatting

Bij 8 multipel myeloompatienten werd in 5 gevallen een longinfectie gevonden. Driemaal ging het om een « pneumopathie révélatrice », tweemaal ora een « pneumopathie secondaire ». Aan de hand van deze vaststelling wordt het probleem van de gebrekkige antilichamenvorming bij multipel myeloom gediscussieerd, en enkele hypothesen aangaande de structuur der paraprotcines geformuleerd.

Steunend op de afwijkingen in het plasmocytair systeem bij multipele myelomatosis en bij agammaglobulinemie, wordt een verwantschap tussen beide aandoeningen gezocht in een gemeenschappelijke oorsprong als reticulosen.
