Essai Clinique D’UN Nouveau Diuretique, La 2-4-7-Triamino-6-Phenylpteridine (Triamterene)

Summary

Triamterene was administered alone or in combination with thiazide to 10 edematous patients referred for failure of diuretic therapy.

Complete clinical recovery occurred in one case. In the other patients, the induced natriuresis allowed a satisfactory control of edema.

Although triamterene is not as powerful a diuretic as thiazides, it nevertheless enhances their natriuretic and diuretic effects. Its main advantage lies in its capacity to reduce potassium excretion. However, it is apparent from balances performed on 4 patients that addition of triamterene docs not necessarily prevent potassium depiction induced by thiazides.

Side effects of triamterene were minimal : in 4 cases urea and creatinine serum levels rose slightly hut returned to control values on discontinuation of therapy.

Triamterene has a distinct value, when added to thiazides, in the treatment of so-called refractory edema.

Le triamtérène a été administré seul ou en association avec des thiazides à 10 patients aedémateux, hospitalisés pour échec de la thérapeutique diurétique usuelle.

Un asséchement complet des œdemes a étéc obtenu dans un cas. Dans les autres cas, la natriurie ohtcnua a permis un contrôle satisfaisant des œcdemes.

Tout en n’égalant pas les thiazides, le triamtérène potentialise leur effet natriurétique et diurétique. Son intérêt principal réside dans son effet d’épargne potassique. Des hilans pratiqués chez 4 maladcs, il ressort toutcfois que l’adjonc-tion dc triamtérène nc prévicnt pas nécessaircmcnt la déplétion potassique liée a 1’usage des thiazides. La grande maniabilité dé ce produit en fait une médication d’appoint précicuse dans le traitement des œdemes dits rcfractaircs.

Les complications liées a l’administration dc triamtérène sont minimes. Dans 4 cas, on a ohscrve une augmentation des taux sanguins de la créatinine et dc l’urcé, alterations qui ont rapidement cédé a l’arrêt du traitement.

Samenvatting

Aan 10 oedemateuze patintên, ons gestuurd ingevolge mislukte diurctische behandeling, werd triamtérène ofwel zuiver ofwel met thiaziden toegediend.

In één geval verdwenen de oedemen. In de 9 anderen was het natriumverlies voldoende om de oedemen te kontroleren. Alhoewel triamtérène niet zo werk-zaam is als de thiaziden, verhoogt hij merkelijk hun natriuretisch en diuretisch effekt.

Zijn hoofdvoordeel is het vermogen tot vermindering der kaliumuitscheiding. Nochtans, uit de uitslagen van kaliumbalansen, vastgesteld hij 4 patiêntcn, hlijkt dat tocvocging van triamterene nict noodzakclijk ccn kaliumdcpletie, vcroorzaakt door thiaziden, voorkomt.

Verwfkkelingen verbonden aan het gchruik van triamtérène zijn miniem. In 4 gevallcn heeft men een lichte stijging van bloedurea en plasmacreatinine bemerkt welke verdwenen zodra de behandeling werd stop gezet.

Triamtérène toegevoegd aan thiaziden is zeer waardevol in de hchandeling van de zg. weerstandhiedende œdemen.
