Insuffisance Renale Aigue Revelatrice D’Un Myelome Multiple

Summary

The authors report a case of multiple myeloma where acute renal failure was the first manifestation of the disease, and followed an acute infectious episode with dehydration, in a 55 year old man.

The patient recovered from this renal episode by the application of a complex therapy, including haemixlialysis. The main interest of this case is the lack of symptoms suggesting the original disease prior as well as after the remission of uraemia; renal function recovered.

It is stressed that acute renal failure occurring in multiple myeloma is usually a fatal complication but this prognosis may be modified by appropriate therapy.

Les auteurs présentent un cas d’insuffisance rénale aiguë révélatrice d’un myélome multiple, installée chez un homme âgé de 53 ans, après un syndrome infectieux fébrile de type grippal suivi de déshydratation.

Le malade a été récupéré par l’application d’un traitement complexe, qui a inclus aussi une séance d’épuration extra-rénale à l’aide du rein artificiel. On remarque chez ce malade le silence clinique de la maladie de fond, tant avant, qu’après la rémission de l’insuffisance rénale aiguë, et le rétablissement de la fonction rénale normale après la récupération.

On souligne le fait (pie l’insuffisance aiguë qui complique la maladie de Kahler a une évolution sévère, fréquemment létale, mais que le pronostic peut être amélioré par une thérapeutique adéquate.

De auteurs stellen een geval van acute nierinsufficiëntie voor, die een veel-vuldig myeloom liet ontdekken. Deze aandoening werd ontdekt bij en patiënt, 53 jaar oud, na een koortsig bcsmcttelijk syndroom gevolgd door waterloosheid.

De zieke geneesde met de toediening van een ingewikkelde behandeling. waaraan een zuivering met tic kunstmatige nier ook werd toegevoegd. Zowel vóór dan na de vermindering van de acute nierinsufficiëntie, hcet’t men de stilstand van de grondziektc waargenomen. De herstclling van een normale nierfunctie werd trouwens ook bevestigd.

Er wordt aangedrongen op het feit dat de nierinsufficiëntie die de ziekte van Kahler volgt, zich door een ernstige, vaak dodelijke, ontwikkcling kenmerkt. Thans kan de prognose verlxrterd worden door een aangepaste behandeling.
