Summary
Longlasting topical application of Gentamicin, in various skin lesions, induced the appearance of a Pseudomonas aeruginosa strain which was resistant to this antibiotic. Being resistant to more than 100 µg/ml of Gentamicin (by disc agar diffusionand broth dilution test tube methods), this strain shows a particular lysotype. The latter may be present in the lesions together with other susceptible lysotypes. The spreading risks of such strains through the hospital wards are serious and question the topical use of major antibiotics. Resistant strans disappeared after antibiotic therapy have been discontinued.

La Gentamicine en application locale prolongée sur des lésions dermatologiques diverses a entraine l’éclosion d’une souche de Pseudomonas résistante à cet antibiotique. Résistante à plus de 100 gamma de Gentamicine déterminée sur gélose et en dilution, cette souche présente un lysotype particulier. Ce dernier peut voisiner sur les plaies avec d’autres lysotypes restant sensibles à cet antibiotique. Les risques de dissémination de souches semblables en milieu hospitalier ne peuvent être négligés et imposent une grande prudence à l’usage en application locale d’antibiotiques majeurs. Les souches résistantes ont spontanément disparu des lésions à l’arrêt de la thérapeutique.
Samenvatting
Bij een lange lokale behandeling van huidletsels ontstond een Pseudomonas stam resistent aan Gentamicine. Bepaald op vaste en vloeibare bodems was deze stam weerstandig aan meer dan 100 µg/ml Gentamicine, en vertoonde ook een eigen lysotype. Deze laatste alsook andere lysotypen van Gentamicine gevoelige kiemen vertoefden in éénzelfde letsel. Er bestaat een groot gevaar, dat deze stammen zich verspreiden in het ziekenhuis daarom zal een gering gebruik van de hoofd antibiotika aangewezen zijn voor lokale doeleinden. Deze resistente stammen verdwenen spontaan bij het stopzetten van de behandeling.
