Summary
Two cases of hyponatraemia with clinical symptoms of water intoxication are reported in diabetic patients treated with oral antidiabetic drugs. Hyponatraemia induced by chlorpropamide is actually well established. In the first case presented, the role of chlorpropamide in the development of hyponatraemia is confirmed by a therapeutic test. A review of the literature about the mode of action of this sulfonylurea is presented.
The second patient developed severe hyponatraemia with water retention after treatment with both saliuretic and biguanide (phenformin). Although in this second case the therapeutic test was less definite, it seems very likely that phenformin played a role in the development of the hyponatraemia. Indeed, an antidiuretic action of biguanides has been previously reported in patients with diabetes insipidus.
Résumé
Deux cas d’hyponatrémie sévère accompagnés de signes cliniques d’intoxication hydrique sont décrits chez des patients traités par antidiabétiques oraux.
Dans le premier cas, le rôle du traitement par chlorpropamide dans le développement de cette hyponatrémie est démontré par une épreuve thérapeutique. Cet effet secondaire du chlorpropamide est actuellement bien connu, et les données de la littérature concernant son mécanisme sont discutées.
Le second patient présenté a développé un tableau clinique sévère d’hyponatrémie à la suite d’un traitement par un biguanide (la phenformine) associé à un diurétique. Si, chez ce patient, le test thérapeutique est moins nettement démonstratif que dans le premier cas, il semble néanmoins très vraisemblable que la phenformine ait joué un rôle dans le développement de l’hyponatrémie; en effet, une action antidiurétique des biguanides a été décrite précédemment chez des patients atteints de diabète insipide.
Samenvatting
Twee gevallen van zware hyponatremie, met klinische tekens van water intoxicatie worden beschreven bij patiënten behandeld met orale antidiabetica. In het eerste geval, wordt de rol van de behandeling met Chlorpropamide in de ont wikkeling van deze hyponatremie aangetoond door een therapeutische test. Dit neveneffect van Chlorpropamide is heden ten dage wel bekend, en een ovcrzicht van de literatuur omirent zijn mechanisme wordt voorgesteld. De tweede patiënt heeft een klinisch tableau ontwikkeld van zware hyponatremie als gevolg van een behandeling met een biguanide (nl. phenformine) geassocieerd aan een diureticum. Alhoewel, bij deze patiënt, de therapeutische test minder demonstratief is dan bij de eerste, is het nogal waarschijnlijk dat de phenformine en rol gespeeld heeft in de ontwikkeling van de hyponatremie; er werd namelijk een antidiuretische aktie van de biguanide beschreven bij patiënten die leden aan diabetes insipidus.
