Combination Chemotherapy In Hodgkin’s Disease At Advanced Stages

Summary

The authors administered a combination chemotherapy with nitrogen mustard, vincristine, procarbazine and prednisone to 37 cases of Hodgkin’s disease at advanced stages.

Very encouraging results were obtained in previously untreated patients: 82.3% remissions with a plateau around 80 % remission and survival durations up to four years. Thus, the cure of such an advanced disease becomes conceivable, but the appearance of secondary cancers is possible.

On the contrary, in previously treated patients, the results are less favourable: only 30% remissions are achieved with a median survival duration of responders around two years. Moreover, the haematologic toxicity is not negligible in this last group.

Résumé

Les auteurs ont traité par une polychimiothérapie associant moutarde à l’azote, vincristine, procarbazine et prednisone, 37 cas de maladie de Hodgkin au stade avancé.

Des résultats très encourageants ont été obtenus chez les sujets non traités antérieurement: 82,3% de rémissions et un plateau dans la durée des rémissions et de la survie jusqu’à plus de 4 ans. Dès lors, la curabilité de la maladie de Hodgkin est concevable même à un stade avancé, mais l’apparition de cancers seconds est possible.

Chez les patients en rechute après un traitement préalable, les résultats sont moins favorables: seulement 30% de rémissions et une durée médiane de survie à 2 ans pour les cas entrés en rémission. La toxicité hématologique est importante dans ce dernier groupe.
