Computer Georienteerd Orthopedisch Medisch Dossier^*

Summary

The intention of this paper is to show a trial of a computeroriented orthopaedic record, on which the first work was started in 1967. The main purpose of our work is storage and retrieval of information, with the help of structured charts which partly replace the old handwritten record.

The first information is gathered in the out-patient clinic where mostly diagnosis and treatment information is collected. The largest part of the record, however, concerns the in-patient, and consists of a questionnaire about general health and orthopaedic problems together with specific charts for different conditions.

Par cette publication nous tâchons de montrer un exemple d’un dossier médical orthopédique automatisé, dont le travail a débuté en 1967.

Le but de notre travail est surtout un stockage et un prélèvement d’informations à l’aide de documents structurés, qui remplacent partiellement le dossier classique.

Les premières informations sont collectées à la consultation externe surtout concernant la diagnostique et le traitement. La plus grande partie du dossier cependant concerne le patient hospitalisé avec un assez volumineux questionnaire, envoyé au patient avant son admission à l’hôpital, et quelques documents structurés, concernant différentes par-ties du système locomoteur.

Deze publicatie tracht een voorbeeld te tonen van een computergeoriënteerd medisch-orthopedisch dossier, waarvoor het eerste werk geleverd werd in 1967.

Onze voornaamste doelstelling is het stockeren en weer ophalen van informatie met behulp van gestructureerde documenten, die gedeeltelijk het klassieke handgeschreven dossier vervangen. De eerste gegevens worden verkregen op de ambulante konsultatie en betreffen vooral de diagnose en behandeling en andere administratieve gegevens.

Het grootste deel van het dossier echter betreft de opgenomen patiënt met een vrij uitgebreide vragenlijst aangaande de algemene gezondheidstoestand en aangaande het algemeen orthopedisch onderzoek.

Verder bevat het dossier specifieke documenten betreffende bepaalde aandoeningen op orthopedisch gebied.
