Abstract
We present a case in which the patient had left-sided chest pain and was found to have an enlarged spleen with a necrotic mass and multiple pulmonary nodules. The splenic biopsy was nondiagnostic and repeat imaging showed rapid enlargement of the pulmonary nodules with high standardized uptake value (SUV) uptake. Spindle cells present on the lung pathology initially led to the diagnosis of sarcomatoid lung cancer. However, further review showed CD20 staining on the specimen and the final diagnosis was diffuse large B-cell lymphoma (DLBCL), spindle cell variant. The patient was successfully treated with rituximab, cyclophosphamide, hydroxydaunorubicin hydrochloride, vincristine and prednisone (R-CHOP) after the diagnosis was confirmed. This is a very rare form of DLBCL that is commonly misdiagnosed and is often not on the differential for spindle cell tumors. To our knowledge, DLBCL spindle cell variant has never presented or been diagnosed from pulmonary nodules.
RÉSUMÉ
Nous présentons un cas dans lequel le patient avait des douleurs thoraciques du côté gauche et s’est avéré avoir une rate élargie avec une masse nécrotique et de multiples nodules pulmonaires. La biopsie splénique était non diagnostique et l’imagerie répétée a montré un élargissement rapide des nodules pulmonaires avec une valeur d’absorption normalisée (SUV) élevée. Les cellules fusiformes présentes sur la pathologie pulmonaire ont initialement conduit à un diagnostic de cancer du poumon sarcomatoïde. Cependant, un examen plus approfondi a montré une coloration au CD20 sur l’échantillon et le diagnostic final a été un lymphome diffus à grandes cellules B (LDGCB), variant à cellules fusiformes. Le patient a été traité avec succès par R-CHOP (rituximab, cyclophosphamide, chlorhydrate d’hydroxydaunorubicine, vincristine et prednisone) après confirmation du diagnostic. Il s’agit d’une forme très rare de LDGCB qui est souvent mal diagnostiquée et qui, souvent, ne fait pas partie du différentiel pour les tumeurs à cellules fusiformes. À notre connaissance, la variante de LDGCB à cellules fusiformes n’a jamais été présentée ou diagnostiquée à partir de nodules pulmonaires.
