Abstract
The benefits of discharge planning have been well-documented. However, there is little description of how nurse educators prepare students for this role. This article describes a model for providing a discharge planning clinical experience to nursing students at the same time as their acute care experience. The benefits, problems, and solutions of the experience are discussed.
