The role of metastasectomy in renal cell carcinoma

Abstract

Despite significant advances in the systemic treatment of metastatic renal cell carcinoma, long-term survival remains low. A potential way to improve outcomes in selected cases is the use of metastasectomy, which is part of the multimodal treatment of this disease. Although the evidence supporting this approach is limited, we believe it is a reasonable option for certain patients. This review summarizes the evidence supporting this approach.

Key issues

  • • Despite significant advances in the systemic treatment of metastatic renal cell carcinoma (RCC), long-term survival remains low.

  • • Metastasectomy forms part of the multimodal treatment of metastatic RCC and appears to improve long-term survival in highly selected patients.

  • • The evidence supporting metastasectomy in RCC is limited with the majority of studies being retrospective and subject to inherent selection bias.

  • • The role of metastasecomy in RCC in the era of targeted therapy remains unclear with more research needed to assess its efficacy and safety.

  • • Currently, the decision of whether a patient should proceed with metastasectomy needs to be assessed on a case-by-case basis by the multidisciplinary team after consideration of the advantages and disadvantages of this approach.
