Levonorgestrel-releasing intrauterine system in contraception

Abstract

The levonorgestrel-releasing intrauterine system (LNG IUS) is a long-acting, reversible, contraceptive method with several noncontraceptive benefits. The contraceptive efficacy is equal to female sterilization in randomized prospective trials as well as in real life, with 5-year failure rates of 0.5–1.1 per 100 users. The absolute rate of ectopic pregnancies is low. However, if a pregnancy begins during the use of the LNG IUS, it is more likely to be ectopic. The LNG IUS is equally effective in all age groups. It can be used for young and nonparous women, and the bodyweight of the user is not associated with failure of the method. The continuance rates of the LNG IUS are good. Premature removal of the system is most often associated with heavy menstrual bleeding and pain. The risk of premature removal is diminished among women who experience total or occasional loss of menstrual bleeding. User-satisfaction is strongly associated with the information given at the time of the LNG IUS insertion.
