Zum Cogan-Syndrom

Abstract

Cogan's Syndrom is the clinical entity of non syphilitic interstitial keratitis with vestibuloauditory symptoms. Although the disease is not rare the patho-histological findings in temporal bones are only reported twice. Our communication deals with observations made on a temporal bone out of O. Mayer's collection. The most striking findings are: the endolymphatic hydrops, the degeneration of the organ of Corti, the pathology of the ductus reuniens, the fibrosis within the cisterna peri-lymphatica, the destruction of the maculae and the most uncommon ossification within the scala vestibuli of the basal turn. The vessels seem to show signes of angiitis.

Zusammenfassung

Es wird über pathohistologische Befunde, erhoben an einem menschlichen Felsenbein beim Cogan Syndrom berichtet. Die hervorstechenden Beobachtungen sind: 1. Fibrose im perilymphatischen Raum: Scala vestibuli, cysterna perilymphatica vestibuli, rund um den oberen Bogengang und den Ductus reuniens. 2. Ossificationen in perilymphatisch-fibrosierten Arealen: Scala vestibuli der basalen Schneckenwindung, Vestibulum. 3. Zerstörung der Sinnesendstellen: Corti'sches Organ macula sacculi und utriculi. 4. Endolymphatischer Hydrops: mittlere, obere Schneckenwindung, Sacculus, Utriculus, oberer Bogengang. 5. Perisacculäre Fibrose im Ductus endolymphaticus und Blockade der Bast'schen Klappe. 6. Panarteriitis nodosa ähnliche Gefäβveränderungen an der Arteria labyrinthi, den Venen um den Aquaeductus vestibuli und an alien Gefäβen um den Nervus facialis. Auf Grund dieser Veränderungen nehmen wir an, daβ es sich beim Cogan Syndrom um eine localisierte Erkrankung aus dem Formenkreis der Autoimmunerkrankungen handelt.
