High-Frequency Audiometry Using Precision Earphones: Reliability under Laboratory and Field Conditions

Abstract

New circumaural earphones were tested in the frequency range from 100 Hz to 20 kHz and compared to commonly used supra-aural earphones. The circumaural earphone HD 230 (Sennheiser) generates test stimuli at frequencies up to 20 kHz with almost constant sound pressure levels when its position is changed on an artifical ear. The reproducibility of hearing threshold measurements tested with a new microprocessor-controlled Békésy audiometer using the HD 230 earphone was nearly as good as under free-field conditions. The practicability and the diagnostic value of high-frequency audiometry have been demonstrated on pupils under field conditions. For this application, the good sound attenuation properties of these earphones (30 dB above 1 kHz) are important. High-frequency hearing thresholds of healthy pupils and of pupils with a history of otitis media are shown. At 16 kHz, the hearing threshold level difference between both groups reached 20 dB.

Un écouteur entourant l'oreille (Sennheiser HD 230) produit des signaux tests sonores de fréquences allant de 100 Hz à 20 kHz, dont le niveau se modifie à peine par déplacement latéral sur l'oreille artificielle. Jusqu'à 16 kHz la reproductibilité des mesures de seuils d'audibilité était presque aussi bonne avec cet écouteur qu'en champ libre. Nous avons pu aussi effectuer sur le terrain (dans une école) des mesures de seuils d'audibilité fiables jusqu'à 16 kHz à l'aide de cet écouteur et d'un audiométre commandé par microprocesseur. Chez un groupe d'écoliers ayant été atteints d'une otite de l'oreille moyenne, la capacité de perception des sons de haute fréquence était dimunuée de façon significative.
