Impedance Audiometry as Screening Method in School Children

Abstract

In order to evaluate the usefulness of impedance audiometry as a screening method for school children, two different pilot studies were performed. The results from screening audiometry were compared with the results fromotoscopy and impedance audiometry (tympanometry and stapedius reflex test). The first study comprised a selected material of 200 ears of 7—year—olds who fell through in screening audiometry at school. To illustrate, we found that the screening audiometry was pathological in 64%, otoscopy in 85.5%, tympanometry in 95.5% and stapedius reflex test in 99%. The second study comprised 206 ears of 10—year—olds randomly selected. In this material we found 1.5% pathological values with screening audiometry, 8% with otoscopy, 12% with tympanometry and 35% with stapedius reflex test. The clinical significance of the raised stapedius reflex threshold as well as of the under—pressure in those ears declared normal with otoscopy has to be determined.

Tvd olika pilotstudier har utförts med avsikt att undersöka impedansaudiometrins användbarhet som screeningsmetod på skolbarn. Resultaten av vanlig screeningsaudiometri har härvid jämförts med resultaten av dels otologisk undersökning och dels av impedansaudiometri (tympanometri och stapediusreflexmätning.) Den första undersökningen omfattade 200 selekterade öron hos 7—åringar som enligt screeningaudiometri i skolan hade nedsatt hörsel. Vid undersökning hos oss 2 veckor senare påvisades patologiska värden vid kontroll med screeningaudiometri i 64%, vid otoscopi i 85.5%, vid tympanometri i 95.5% samt vid stapediusreflexmätning i 99%. Den andra undersökningen utfördes i skolan och omfattade 206 ej selekterade öron hos 10—åringar. I detta material påvisades patologiska vården med screeningaudiometri i 1,5%, vid otoscopi i 8%, vid tympanometri i 12% samt stapediusreflexmåtning i 35%. Diskrepansen mellan otologens resultat och resultatet av impedansaudiometrin återstår att utvaUrdera kliniskt.
