Influence of the adrenergic nervous system on the lower urinary tract and its clinical implications

Abstract

Summary

The morphology of the adrenergic nervous system as well as adrenoceptor functions of the human lower urinary tract are outlined and compared to some animal studies. Special emphasis is given to the altered adrenergic innervation and adrenoceptor function after parasympathetic decentralization. Pharmacological treatment affecting adrenoceptors is described in different disturbances of lower urinary tract function and some evidence for adrenergic mechanisms working in intravaginal electrical stimulation is presented. The importance of the sympathetic nervous system for the normal function of the human lower urinary tract is still unclear. Nevertheless, clinical data obtained by treatment with agents influencing this system indicate that the contractory α-adrenoceptors of the human urethra can be stimulated or blocked pharmacologically producing significant changes in intraurethral pressure. On the other hand, the relaxatory effect on the human detrusor obtained by stimulation of the β-adrenoceptors seems to be negligible.

Résumé

La morphologie du système nerveux adrénergique et les fonctions des adrénocepteurs de la voie urinaire inférieure humaine sont brièrement décrites et comparées à des études réalisées sur des animaux. L'article insiste plus particulièrement sur la modification de l'innervation adrénergique et de la fonction des adrénocepteurs après décentralisation parasympathique. Un traitement pharmacologique affectant les adrénocepteurs est décrit dans différents cas de perturbation de la voie urinaire inférieure; l'article cite certains cas montrant des mécanismes adrénergiques fonctionnant sous stimulation électrique intravaginale. L'importance du système nerveux sympathique dans la fonction normale de la voie urinaire inférieure reste difficile à déterminer. Toutefois, les données médicales obtenues à partir de traitements utilisant des agents influencant ce système indiquent que les adrénocepteurs de contraction alpha de l'urètre humaine peuvent ětre stimulés ou bloqués pharmacologiquement, causant ainsi des modifications notables de la pression intraurétrale. Au contraire, l'effet de relaxation sur le détruseur humain obtenu par stimulation des adrénocepteurs běta apparaiˇt négligeable.

Zusammenfassung

Die Morphologie des adrenergischen Nervensysteme, sowohl als die Adrenoceptor Funktionen im menschlichen unteren Harntrakt werden hier beschrieben und mit Tierversuchen verglichen. Man gibt besonderen Nachdruck auf die veränderte adrenergische Innervation und adrenoceptor Funktion nach parasympathischer Dezentralisierung. Pharmakologische Behandlung, welche Adrenoceptoren beeinflusst, wird beschrieben, und zwar bei verschiedenen Störungen der unteren Harntraktfunktion, und man bringt Beweise, dass adrenergische Mechanismen bei intravaginaler Stimulation funktionieren. Die Bedeutung des sympathischen Nervensystems bei der normalen Funktion des menschlichen unteren Harntrakts ist noch nicht ganz klar. Jedoch zeigen klinische Daten, die durch Behandlung mit Mitteln, welche dieses System beeinflussen, gewonnen wurden, dass die verengenden Alpha-Adrenoceptoren des menschlichen Urethers pharmakologisch stimuliert oder gehemmt werden können, und dadurch wesentliche Veränderungen im intraurethralen Druck hervorgerufen werden. Andererseits scheint die entspannende Wirkung auf den menschlichen Detrusor, die man durch Stimulierung des Beta-Adrenoceptors erhält, unwesentlich.

It is with great sadness that we have to inform the readership of the Journal that shortly after completing the final editorial work on this Special Issue, Professor Kalman Jacob Mann was seriously injured in a car accident and subsequently died.

Professor Mann was responsible for establishing the two Hadassah Hospitals and Community Health Centres in Jerusalem and for the past 20 years headed the Presidium of Yad Sarah, Israel's largest community based, volunteer operated organization which provides a spectrum of free or nominal cost home care services nation-wide.

We offer our condolences to his family and friends, and trust that this Special Issue stands as a testament to his work in the field.
