The place of amputation in the management of the ischaemic lower limb in the dysvascular geriatric patient

Abstract

Summary

Statistics show that today the majority of amputations of the leg are performed for peripheral vascular disease in patients over the age of 60. The stages of management are discussed and in particular a plea made for taking a balanced view of vascular salvage procedure and amputation as alternative treatments. The advantages are stressed for these patients to be treated in a unit where the resources for appropriate investigation are available and the surgeon has the support of a multi-disciplinary team. The importance of the pre- and post-amputation rehabilitation programme is discussed.

Résumé

Les statistiques confirment que la plupart des amputations aux membres inférieurs sont actuellement pratiquées chez des personnes àgées de plus de 60 ans et qui présentent une insuffisance vasculaire périphérique. L'auteur aborde les divers éléments du plan thérapeutique et incite à adopter une attitude nuancée et réfléchie quant au choix entre le recours à la chirurgie vasculaire réparatrice ou à l'amputation. Il souligne l'importance de la prise en charge des patients dans une unité specialisée qui dispose de tout l'équipement nécessaire pour réaliser les investigations appropriées et où le chirurgien peut obtenir une bonne collaboration inter-disciplinaire. On insiste sur l'importance d'un programme de réhabilitation médicale bien structuré, aussi bien avant qu'après l'amputation.

Zusammenfassung

Statistiken zeigen, dass die Mehrzahl der Amputationen im Bereich der unteren Extremitäten wegen peripherer Gefässerkrankungen bei über 60 Jahre alten Patienten durchgeführt wird. Die Stadien der Behandlung werden diskutiert, und es wird ein Plädoyer für eine ausgeglichene Balance von konservierenden und wiederherstellenden gefässchirurgischen Eingriffen und der Amputation als Alternativ-Behandlung vorgetragen. Besonders herausgehoben werden die Vorteile, die sich für diejenigen Patienten ergeben, die in einer Spezialabteilung behandelt werden können, in welcher die Möglichkeiten geeigneter Untersuchungen zur Verfügung stehen und der Chirurg die Unterstiitzung eines multidisziplinären Teams geniesst. Die Bedeutung der prä- und postoperativen Rehabilitationsprogramme um die eigentliche Amputation herum wird diskutiert.

La estadística demuestra que, en la actualidad, la mayoría de las amputaciones de las extremidades inferiores se deben a las vasculopatías periféricas de pacientes de más de 60 años de edad. Se comentan los estadios del tratamiento, y en especial se propugna un punto de vista equilibrado en lo que respecta a los métodos de rescate vascular y de amputatión como tratamientos alternativos. Se recalcan las ventajas para aquellos pacientes a tratar en una unidad en que se cuenta con recursos para una investigatión adecuada, y en que el cirujano cuenta con el apoyo de un equipo con disciplinas múltiples. Se comenta la importancia del programa de rehabilitatión antes y después de las amputaciones.

It is with great sadness that we have to inform the readership of the Journal that shortly after completing the final editorial work on this Special Issue, Professor Kalman Jacob Mann was seriously injured in a car accident and subsequently died.

Professor Mann was responsible for establishing the two Hadassah Hospitals and Community Health Centres in Jerusalem and for the past 20 years headed the Presidium of Yad Sarah, Israel's largest community based, volunteer operated organization which provides a spectrum of free or nominal cost home care services nation-wide.

We offer our condolences to his family and friends, and trust that this Special Issue stands as a testament to his work in the field.
