Hypertension and Renal Function in Diabetes Mellitus—A Review

Abstract

Hypertension is a regular accompaniment of diabetes mellitus (DM), especially Type I, insulin-dependent diabetes mellitus (IDDM) (1, 2). Hypertension appears in the early stages of IDDM, as the GFR begins to decrease though it is still within the normal range (3). The onset of hypertension coincides with the onset of microalbuminuria (4). Even though BP may still be within the so-called normal range, it is higher in patients with higher urine albumin excretion (UAE): BP = 128/79 ′ 13/6 mm Hg in IDDM patients with UAE < 20 μg/m; 135/86 ′ 14/7 in patients with UAE 20-200 μg/m, and 147/93 ′ 21/11 mm Hg in patients with UAE > 200 μg/m (5).
