The Gateway Hypothesis of Substance Abuse: An Examination of Its Applicability in the Nigerian General Population

Abstract

The study aims to estimate the prevalence and predictors of not following the gateway theory. Respondents were selected from a multistage stratified clustered sampling of households in five of Nigeria's six geopolitical regions. Interviews were conducted between February 2002 and May 2003 using the CIDI-version.3 with a total sample size (N) of 2,143. Cumulative incidence proportions of not following a gateway pattern were estimated with SUDAAN. Predictors of this were estimated using multivariate logistic regression models. The deviation from the normative sequence of drug use occurs albeit infrequently. The public health implications of this are discussed as well as the limitations of the findings.

L’Hypothèse de Porte d’Abus de Substance: un Examen de sa Validité d’application dans la Population Générale nigériane

l’étude vise à estimer la prévalence et les prédicteurs de ne pas suivre la “Gateway” théorie. Les répondants ont été choisis d’un multi-stades stratifié clusterisé échantillonnage des ménages dans cinq du Nigéria s six régions géopolitiques. La Hipótesis de la Puerta de Abuso de Sustancia: un Examen de su Aplicabilidad en la Población General nigeriana

El estudio pretende estimar la prevalencia y factores predictivos de no siguiente la teoría de entrance de consumo de drogas. Los encuestados fueron seleccionados de un muestreo polietápico estratificado agrupadas en cinco de los hogares de Nigeria seis regiones geopolíticas. Dr. Bibilola Damilola Oladeji is a fellow of the West African College of Physicians, a lecturer in the Department of Psychiatry, College of Medicine, University of Ibadan, and a consultant psychiatrist to the University College Hospital, Ibadan, Nigeria. Research interests include epidemiology, rehabilitation psychiatry, and mental health aspects of physical disorders.

Oye Gureje, M.B.B.S., Ph.D., D.Sc., F.R.C.Psych., F.R.A.N.Z.C.P., F.W.A.C.P., is a professor and the head of the Department of Psychiatry at the University of Ibadan, Nigeria, and a consultant psychiatrist at the University College Hospital, Ibadan, Nigeria. He received his medical training in Nigeria and postgraduate training in both Nigeria and England. He was for several years a senior psychiatrist at the Department of Psychiatry, Royal Park Hospital, Parkville, Victoria, Austarila, and an honorary associate professor in the Department of Psychiatry, University of Melbourne, Australia. He is currently the president of the Association of Psychiatrists in Nigeria.

Dr. Victor Adesola Makanjuola, M.B.B.S., M.Sc., F.W.A.C.P., is a lecturer in the Department of Psychiatry, University of Ibadan, and a consultant psychiatrist at the University College Hospital, Ibadan. He attended the University of Ibadan for his medical education, which he completed meritoriously in 1998. His major research interests are in addiction psychiatry and community psychiatry.
