The Treatment of Nodose Tendinitis in the Rheumatoid Hand

Summary

The authors compare the treatment of nodose tendinitis in the rheumatoid hand by (a) local corticosteroid injections and (b) surgery involving opening of the tendon sheath and possible removal of tendon nodules. They set down their principles for having recourse to either procedure. Cases of muscle balance disturbance should be treated in collaboration with a hand surgeon. If the impaired function is chiefly ascribable to changes in the flexor tendon, a series of cortisone injections seems indicated, but requires continuous control. If the results of local steroid injections are poor, surgery should be considered without unnecessary delay.

Les auteurs comparent le traitement dé la tendinite nodeuse dans la main rhumatismale par (a) injections corticostéroïdes locales et par (b) intervention chirurgicale consistant à ouvrir la gaine du tendon et à éliminer éventuellement la nodule. Ils font un compte rendu de leurs principes pour recourir à chacun de ces procédes. Des cas de dérangement dans l'équilibre musculaire devraient être traités en collaboration avec un chirurgien des mains. Si la fonction troublée est attribuable principalement à des altérations dans le tendon fléchisseur, une série d'injections de cortisone semble être indiquée, mais il faudra soumettre le malade à un contrôle continuel. Si le traitement ne donne pas de résultats satisfaisants l'intervention chirurgicale devra être prise en considération sans tarder inutilement.

Zusammenfassung

Die Verfasser vergleichen die Behandlung der nodösen Tendinitis in der rheumatischen Hand durch (a) örtliche Kortikosteroid-Einspritzungen und (b) chirurgische Eingriffe, darin bestehend, dass man die Sehnenscheiden öffnet und möglicherweise die Sehnenknötchen entfernt. Die Verfasser erklären ihre Grundsätze betreffend die Anwendung der beiden Behandlungsarten. Fälle von Muskelgleichgewichtsstörungen sollten in Zusammenarbeit mit einem Handchirurgen behandelt werden. Wenn die gestörte Funktion hauptsächlich auf Veränderungen der Beugesehne zurückzuführen ist, scheint eine Serie von Kortisoneinspritzungen angezeigt zu sein, jedoch erfordert diese Behandlung eine fortgesetzte Überwachung. Sollte diese Behandlung keine zufriedenstellenden Ergebnisse zeigen, dürfte ein chirurgischer Eingriff ohne unnötigen Verzug angezeigt sein.
