Summary

The author points out that the impression given by Binder et alii that a gluten-free diet is likely to be of no value in treating rheumatoid arthritis is hardly justified by their study in which a gluten-free diet of a mere two or three weeks was administered to five patients only, three of whom have shown symptoms of the disease for 10 years or more — one of them for 52 years.

The present writer concedes that the benefits of this diet in rheumatoid arthritis have yet to be scientifically confirmed and that the jejunum portion of the small intestine (an organ of some seven metres with five million villi and ten square metres of absorptive surface) appears to be normal on peroral biopsy and other tests.

The author points out, however, that the Yale report of Binder et alii in criticizing the author's geographic correlations of the prevalence of rheumatoid arthritis with the dietary habits based on a particular staple, contains some faulty generalizations, which the author attempts to correct by citing the more recent epidemiological studies of rheumatoid arthritis among the non-European populations.

L'auteur signale que l'impression donnée par Binder et alii qu'une diète exempte de gluten semble être sans valeur dans le traitement de la polyarthrite chronique évolutive est à peine justifiée par leur étude dans laquelle un régime exempt de gluten durant à peine deux à trois semaines a été administré à cinq patients seulement dont trois avaient présenté des symptômes de la maladie durant 10 ans ou plus et l'un d'eux durant 52 ans.

Zusammenfassung

Der Verfasser weist darauf hin, dass der Eindruck, den Binder et alii verbreiten, wonach eine Glutenfreie Diät anscheinend wertlos sei bei der Behandlung der chronischen Gelenkentzündung, durch ihre Untersuchung kaum berechtigt ist, in welcher eine Glutenfreie Diät von bloss zwei bis drei Wochen an nur fünf Patienten verabreicht wurde, von denen drei Patienten Symptome der Krankheit vor zehn Jahren oder mehr aufwiesen und einer von ihnen während 52 Jahren an dieser Krankheit gelitten hatte.
