Dysplasia Epiphysealis Hemimelica: An Unusual Case Combined with Juvenile Rheumatoid Arthritis

Summary

Dysplasia epiphysealis hemimelica is an example of a primary epiphyseal growth disturbance. The condition is very rare, but it has been described with increasing frequency in recent years. Together with a review of the literature available and a summary of the usual and more unusual features of the disease, a case involving symmetrical expansive affections of both lower extremities, and especially affections of the patellae, calcanei, possibly one shoulder and the sacroiliac joints, is presented, hence comprising localisations not previously described. The present case illustrates the fact that the disorder may occur as a systemic disease in the growth zones and epiphyses of the bones, and in this clearly distinguish themselves from ordinary osteochondromas. The patient, a 12-year-old boy, was also suffering from rheumatoid arthritis. The potential significance of the rheumatoid illness for the cause of the epiphyseal disturbances, or possibly as a factor in provoking a latent embryopathy, is discussed, in connection with various etiological considerations.

La dysplasie épiphysaire hémimélique est un exemple de croissance anormale du cartilage épiphysaire. Le cas se présente très rarement mais il a été décrit plus fréquemment ces dernières années. En combinaison avec une revue de la littérature disponible et un résumé des conditions typique et atypique de la maladie un cas comprenant une affection symétrique expansive des deux extrémités inférieures et spécialement l'affection des rotules, des calcanei, possiblement localisée à une épaule et aux articulations sacro-iliaques est présentée, comprenant des localisations non décrites jusqu'ici. Le cas décrit illustre le fait que le trouble peut être considéré comme une maladie systémique affectant les zones de croissance et les épiphyses des os se distinguant clairement en ce sens des ostéochondromes ordinaires. Le patient, un garçon Âgé de douze ans, souffrait également de polyarthrite chronique évolutive. L'auteur discute la signification potentielle de l'affection rhumatoïde pour la cause des troubles épiphyséaux, ou possiblement comme facteur provoquant une embryopathie latente, en connexion avec différentes considérations étiologiques.

Zusammenfassung

Dysplasia epiphysealis hemimelica ist ein Beispiel einer primären epiphysealen Wachstumsstörung. Dieser Zustand ist sehr selten, wurde jedoch in den letzten Jahren immer häufiger beschrieben. In Verbindung mit einem Rückblick auf die zur Verfügung stehende Literatur und einer Zusammenfassung der gewöhnlichen und ungewöhnlicheren Merkmale der Krankheit, wird ein Fall in Zusammenhang mit symmetrischen, expansiven Erkrankungen beider unterer Extremitäten und vor allem mit Erkrankung der Kniescheiben, Fersenbeine, möglicherweise einer Schulter und der Sakroiliakalgelenke gezeigt, unter Einbeziehung von bisher nicht beschriebenen Lokalisierungen. Der vorliegende Fall veranschaulicht die Tatsache, dass die Störung als Systemerkrankung in den Wachstumszonen und den Epiphysen der Knochen auftreten kann und sich hierin deutlich von gewöhnlichen Osteochondromen unterscheidet. Der Patient, ein 12 Jahre alter Knabe, litt auch an juveniler rheumatoider Polyarthritis. Die potentielle Bedeutung dieser Erkrankung als Ursache der epiphysealen Störungen, oder möglicherweise als ursächlicher Faktor einer latenten Embryopathie wird in Verbindung mit verschieden ätiologischen Erwägungen besprochen.
