Uveal Changes in Spondylitis Ankylopoetica

Summary

A total of 93 patients suffering from spondylitis ankylopoetica for iridocyclitis were examined. Uveitis was found in 32 patients, i. e., in 35 % of cases. In 13 patients iridocyclitis had occurred prior to the manifestations of spondylitis. In 19 patients spondylitis had manifested itself prior to the occurrence of iridocyclitis. 25 patients (27 %) had experienced recurrent iridocyclitis.

In view of the frequent affection of the eye in spondylitis ankylopoetica, it cannot be overemphasized that the ophthalmologist must consider spondylitis whenever he meets with iritis, particulary recurrent iritis, especially if there is a history of sciatica. It is, therefore, imperative that in such cases rheumatological examination and radiological investigation of the sacroiliac joints and the spine be carried out.

Résumé

93 malades atteints de spondylarthrite ankylosante ont été examinés concernant l'irido-cyclite. Les auteurs constatèrent une uvéite chez 32 patients c.-à-d. dans 35 % des cas. Chez 13 patients une irido-cyclite survint avant les manifestations de spondylarthrite. Chez 19 patients la spondylite s'est présentée avant l'occurence de l'irido-cyclite. 25 patients (27 %) avaient été atteints d'irido-cyclite récurrente.

Etant donné la fréquence de l'affection des yeux dans les cas de spondylarthrite ankylosante, il est d'une importance primordiale que l'ophtalmologiste tienne compte de la présence de spondylite lorsqu'il constate l'existence d'iritis, notamment d'iritis récurrente et spécialement s'il est question de sciatique dans l'historique de la maladie. Dans de pareils cas il est, par conséquent, d'une nécessité impérieuse de soumettre le patient à un examen rhumatologique et radiologique des articulations sacro-iliaques et de la colonne vertébrale.

Zusammenfassung

Insgesamt 93 Patienten mit Wirbelsäulenversteifung wurden auf Iridozyklitis untersucht. Uveitis wurde bei 32 Patienten festgestellt, also in 35 % der Fälle. Bei 13 Patienten hatte sich Iridozyklitis vor Auftreten der Symptome einer Spondylitis gezeigt. Bei 19 Patienten zeigte sich Spondylitis vor dem Auftreten der Iridozyklitis. Bei 25 Patienten (27 %) war rückläufige Iridozyklitis belegt.

In Hinblick auf die häufig auftretende Erkrankung des Auges bei Wirbelsäulenversteifung kann nicht eindringlich genug darauf hingewiesen werden, dass der Augenarzt eine Spondylitis in Betracht ziehen muss, sobald er Iritis — besonders rückläufiger Iritis — begegnet, vor allem wenn eine Krankengeschichte vorliegt, die Ischias zeigt. In derartigen Fällen ergibt sich daher zwingend die Notwendigkeit, eine rheumatologische und radiologische Untersuchung der Ileosakralgelenke und der Wirbelsäule vorzunehmen.
