Summary
Erosive changes of the sternoclavicular joints were demonstrated with the aid of tomography in 30 per cent of 105 patients suffering from rheumatoid arthritis. The rate of degenerative changes increasing after 40 years of age was also about 30 per cent. Both conditions were often seen simultaneously.
The frequency of degenerative arthrotic changes in a control group of 50 cases was 40 per cent.
The sternoclavicular joint was affected in only 5 per cent in the beginning of the rheumatoid arthritis, while this joint was involved in 30 to 40 per cent after a duration of 1 year.
The only dependable roentgenologic changes indicative of rheumatoid arthritis were erosions best demonstrable with the aid of tomography.
Résumé
Chez 30 pourcent de 105 malades souffrant de polyarthrite chronique évolutive les auteurs ont pu démontrer des altérations érosives des articulations sterno-claviculaires à l'aide de la tomographie. Le taux de malformations dégénératives augmentant après l'âge de 40 ans était également d'environ 30 pourcent. Les deux conditions ont souvent été constatées simultanément.
La fréquence des altérations arthrotiques dégénératives était de 40 pourcent dans un groupe de contrôle de 50 cas.
L'articulation sterno-claviculaire n'était affectée que chez 5 pourcent de malades au début de la polyarthrite chronique tandis que cette articulation était atteinte dans 30 à 40 pourcent des cas après une durée d'un an de l'affection.
Les seules altérations radiologiquement décelables de la polyarthrite chronique évolutive étaient les érosions démontrables à l'aide de la tomographie.
Zusammenfassung
Bei 105 Patienten, die an chronischer Gelenkentzündung (RA) litten, wurden mit Hilfe der Tomographie bei 30 Prozent erosive Veränderungen der sternoklavikularen Gelenke aufgezeigt. Der Prozentsats an degenerativen Veränderungen, der im Alter von über 40 Jahren zunahm, lag gleichfalls bei ungef. 30 Prozent.
Bei einer Kontrollgruppe, die 50 Fälle umschloss, lag die Häufigkeit degenerativer arthrotischer Veränderungen bei 40 Prozent.
Das sternoklavikulare Gelenk war bei beginnender chronischer Gelenkentzündung nur bei 5 Prozent angegriffen, wogegen es nach einem Jahr bei 30 bis 40 Prozent angegriffen war.
Die einzigen verlässlichen, röntgenologischen Veränderungen, die auf chronische Gelenkentzündung hinwiesen, waren Erosionen, die am besten mit Hilfe der Tomographie gezeigt werden konnten.
