Summary
13 gonarthritic patients treated with epiphyseal stapling of the knee are reported. Four of them had possible and 9 definite rheumatoid arthritis, and the operation indications were anisomelia or valgus deformity. No serious complications were seen. With follow-up from 1 to 10 years after operation and removal of staples, only one had persistent valgus deviation, one persistent anisomelia and three flexion contractures 10—15°. Ten had complete function without any deformity. The importance of careful postoperative checking is stressed. Epiphyseal stapling of the knee in anisomelia in juvenile rheumatoid arthritis is not recommended. On the other hand, valgus deformity seems to be an important indication for epiphyseal stapling even at an early age.

Le rapport concerne 13 malades souffrant de gonarthrite traités par agrafage épiphyséal de genou. Quatre d'entre eux souffraient de polyarthrite chronique évolutive probable et cette affection définie et les indications opératoires étaient l'anisomélie ou une difformité valgus. Aucune complication sérieuse ne fut constatée. Après une période postopératoire de 1 à 10 années et l'enlèvement des agrafes un seul patient conserva une déviation valgus, un autre une anisomélie persistante et trois autres des contractures en flexion de 10 à 15°. Dix patients avaient un parfait fonctionnement sans aucune difformité. Les auteurs insistent sur l'importance d'un contrôle postopératoire consciencieux. Ils ne recommandent pas l'agrafage épiphyséal du genou dans les cas d'anisomélie et de polyarthrite chronique évolutive juvénile. D'autre part la difforrnité valgus semble être une indication importante pour l'opération d'agrafage épiphyséal même à un âge précoce.
Zusammenfassung
Es wird von 13 Patienten mit Gonarthritis berichtet, die durch epiphysisches Klammern des Knies behandelt wurden. Vier von ihnen litten wahrscheinlich an chronischer Gelenkentzündung, bei neun stand dieser Befund fest. Bestimmend für die Operation war Anisomelie oder Deformation durch Verkrümmung. Es konnten keine ernstlichen Komplikationen festgestellt werden. Bei Beobachtung der Patienten während eines bis zu zehn Jahren nach der Operation und Entfernung der Klammern zeigte nur ein Patient eine hartnäckige Verkrümmung, ein anderer fortdauernde Anisomelie und drei Beugekontrakturen zwischen 10 bis 15°. Bei zehn hatte man volle Funktionstauglichkeit ohne jede Verformung erreicht. Die Wichtigkeit einer sorgsamen postoperativen Kontrolle wird hervorgehoben. Ein epiphysisches Klammern des Knies bei Anisomelie in Verbindung mit juveniler chronischer Gelenkentzündung wird nicht empfohlen. Andrerseits scheint Deformation durch Verkrümmung ein wichtiges Merkmal für epiphysisches Klammern auch in frühem Alter zu sein.
