The goal of surgical therapy in patients with advanced chronic obstructive pulmonary disease is to prolong life by (1) preventing secondary complications, (2) relieving dyspnea, and (3) enhancing quality of life by improving functional status. In this article, the authors review the newer procedure known as lung volume reduction surgery as well as the established lung transplantation technique. Patient selection, complications, and outcome are discussed.
