Recently, the remarkable progress of medical science in treatment of HIV and AIDS has been found to have a price tag attached. Physicians are encountering a litany of heretofore-unrecognized metabolic abnormalities and bizarre body-composition changes in patients receiving the antiretroviral therapy that has so greatly improved their lives. In this article, Dr Cohan describes changes that have been reported and identifies potential pathophysiologic mechanisms. In addition, he offers insight into treatment of this syndrome, which is only beginning to be recognized and studied.
