Geç Başlangıçlı Psikiyatrik Tabloların Ayırıcı Tanısında Nörosifiliz: Olgu Sunumları

ÖZET

Nörosifiliz, bir spiroket olan Treponema Pallidum'un sebep olduğu sifilizin klinik bir formudur. Antibiyotiklerin yaygın kullanımı, sifilizin doğal seyrini değiştirerek geç dönem nörosifilizin farklı klinik görünümlerde ortaya çıkmasına sebep olmuştur. Nörosifiliz, nöropsikiyatrik belirtilerle kendini gösterebilir ve psikoz, deliryum, kişilik değişiklikleri, duygudurum bozuklukları gibi bir çok psikiyatrik bozukluğu taklit edebilir.

Duygudurum belirtileri belirgin geç başlangıçlı psikiyatrik tablo sebebiyle sifiliz serolojisinin araştırıldığı iki olgu sunulmuştur. İlk olguya “kesin nörosifiliz” ikinci olguya “olası nörosifiliz” tanısıyla penisilin tedavisi uygulanmış ve klinik cevap elde edilmiştir. Bu yazı, geç başlangıçlı psikiyatrik hastalıkların ve atipik klinik tabloların ayırıcı tanısında nörosifilizin önemini vurgulamaktadır.

ABSTRACT

Neurosyphilis is a form of tertiary syphilis infection caused by the spirochete bacterium Treponema Pallidum. Frequent use of antibiotics has altered the natural course of syphilis and late-stage neurosyphilis has begun to be seen in different clinical forms. Neurosyphilis may present with neuropsychiatric symptoms and may mimic virtually any psychiatric disorder, such as psychosis, delirium, personality changes and mood disorders.

Here, we present two patients investigated for syphilis serology because of late-onset psychiatric illness with predominant mood symptoms. The first case was diagnosed as “confirmed” neurosyphilis and the second case as “presumptive” neurosyphilis. They were given penicillin treatment and marked improvement in the clinical picture was observed.

This report shows the importance of neurosyphilis in the differential diagnosis of late onset psychiatric disorders and atypical clinical presentations.
