Şizofrenide Kombinasyon Tedavileri

ÖZET

Pek çok tedavi klavuzu şizofrenide monoterapiyi önermesine karşın, klinik pratikte birden fazla antipsikotik kullanımı, özellikle tedaviye dirençli şizofrenide sık karşılaşılan bir durumdur. Literatürde, tedaviye dirençli şizofrenide antipsikotik ilaçların kombine kullanımını destekleyen çok az kanıta rastlanmaktadır. Kombinasyon düşünülmeden önce, herhangi bir monoterapi maksimum tolere edilebilen doza kadar yükseltilmeli ve klozapin dahil en az üç farklı etkinlikte/tolerabilitede olan antipsikotik ilacın monoterapisi denenmelidir. Bu yazının amacı, antipsikotikler ve diğer ajanların kombinasyonunu mevcut literatür temelinde gözden geçirmek, bu girişimleri destekleyen temelleri ve riskleri tartışmak ve bunların klinik pratikteki uygunluklarına değinmektir. Tedaviye dirençli şizofrenide kombinasyon seçeneklerine güvenilir kanıtlar sağlamak için daha fazla çift kör rastgele hasta seçimli kontrollü ve karşılaştırmalı çalışmalara gereksinim vardır.

ABSTRACT

Use of multiple antipsychotic agents in patients with schizophrenia, especially in treatment resistant schizophrenia, is common practice in psychiatry, although most schizophrenia guidelines recommend monotherapy. There is limited evidence to support the combination of multiple antipsychotics in treatment resistant schizophrenia. Any monotherapy should be given at the maximum tolerated dose and at least three antipsychotics with different actions, including clozapine, should be given before a combination is considered. The objectives of this paper are to review the current literature regarding combination of antipsychotics and other agents, as well as to discuss the literature supporting antipsychotic polypharmacy, its risks and its relevance for clinical practice. More randomized controlled trials and head-to-head-comparison trials are necessary to provide substantial evidence and to suggest combination strategies for treatment-resistant schizophrenia.
