* This paper has its origins in the author’s experience as a Fulbright scholar at the University of Canterbury in 1986. He is grateful to the New Zealand-United States Educational Foundation, the University of Canterbury, and the University of Pennsylvania for their support during this period. Early versions of these ideas were presented in talks at the University of Auckland, Victoria University of Wellington, and the University of Canterbury.
