ABSTRACT
Fibromyalgia syndrome (FMS) is a multifaceted and incapacitating functional pain syndrome characterized by continuous, severe, widespread musculoskeletal pain. FMS is associated with other symptoms such as fatigue, nonrestorative sleep, cognitive/emotional dysfunction, and diminished health-related quality of life. The pathogenesis of FMS is still not fully understood, but an increasing amount of evidence supports the link between childhood/adulthood emotional, physical, sexual abuse or neglect and the development of FMS. Managing and treating FMS patients can be challenging because the syndrome is refractory to most treatments. However, psychological interventions, particularly hypnotherapy, have been shown to be effective in the cognitive modulation of fibromyalgic pain. FMS patients may benefit from hypnotherapy alone or in combination with standard medical therapy. Symptom-oriented hypnosis aims to reduce pain, fatigue, sleep problems, anxiety, and depression, while hypnotherapy focuses on resolving emotional conflicts and unresolved traumas associated with FMS. In conclusion, hypnosis may be a useful and safe adjunct tool for managing chronic pain and dysfunctional symptoms in challenging fibromyalgic patients.

Zusammenfssung: Das Fibromyalgie-Syndrom (FMS) ist ein vielschichtiges und beeinträchtigendes funktionelles Schmerzsyndrom, das durch anhaltende, starke, ausgedehnte Schmerzen im Bewegungsapparat gekennzeichnet ist. FMS ist mit anderen Symptomen wie Müdigkeit, nicht erholsamem Schlaf, kognitiver/emotionaler Dysfunktion und verminderter gesundheitsbezogener Lebensqualität (HRQOL) verbunden. Die Pathogenese von FMS ist immer noch nicht vollständig geklärt, aber es gibt immer mehr Belege für den Zusammenhang zwischen emotionalem, körperlichem, sexuellem Missbrauch oder Vernachlässigung im Kindes-/Erwachsenenalter und der Entwicklung von FMS. Die Betreuung und Behandlung von FMS-Patienten kann eine Herausforderung darstellen, da das Syndrom auf die meisten Behandlungen nicht anspricht. Es hat sich jedoch gezeigt, dass psychologische Interventionen, insbesondere Hypnotherapie, bei der kognitiven Modulation von Fibromyalgie-Schmerzen wirksam sind. FMS-Patienten können von einer Hypnotherapie allein oder in Kombination mit einer herkömmlichen medizinischen Therapie profitieren. Die symptomorientierte Hypnose zielt darauf ab, Schmerzen, Müdigkeit, Schlafprobleme, Ängste und Depressionen zu reduzieren, während sich die Hypnotherapie auf die Lösung emotionaler Konflikte und ungelöster Traumata im Zusammenhang mit FMS konzentriert. Zusammenfassend lässt sich sagen, dass Hypnose ein nützliches und sicheres Zusatzinstrument zur Behandlung chronischer Schmerzen und dysfunktionaler Symptome bei anspruchsvollen Fibromyalgie-Patienten sein kann.

Résumé: Le syndrome fibromyalgique (SFM) est un syndrome douloureux fonctionnel à multiples facettes et invalidant caractérisé par une douleur musculo-squelettique continue, sévère et généralisée. Le SFM est associé à d’autres symptômes tels que la fatigue, un sommeil non réparateur, un dysfonctionnement cognitif/émotionnel et une diminution de la qualité de vie liée à la santé. La pathogenèse du SFM n’est pas encore entièrement comprise, mais un nombre croissant de preuves étayent le lien entre l’abus émotionnel, physique, sexuel ou la négligence pendant l’enfance/l’âge adulte et le développement du SFM. La gestion et le traitement des patients atteints du SFM peuvent être difficiles, car le syndrome est réfractaire à la plupart des traitements. Cependant, les interventions psychologiques, en particulier l’hypnothérapie, se sont révélées efficaces dans la modulation cognitive de la douleur fibromyalgique. Les patients atteints du SFM peuvent bénéficier de l’hypnothérapie seule ou en association avec un traitement médical standard. L’hypnose axée sur les symptômes vise à réduire la douleur, la fatigue, les problèmes de sommeil, l’anxiété et la dépression, tandis que l’hypnothérapie se concenter sur la résolution des conflits émotionnels et des traumatismes non résolus associés au syndrome de la fibromyalgie. En conclusion, l’hypnose peut être un outil complémentaire utile et sûr pour gérer la douleur chronique et les symptômes dysfonctionnels chez les patients fibromyalgiques difficiles.

Resumen: El síndrome de fibromialgia (FMS) es un síndrome de dolor funcional multifacético e incapacitante caracterizado por dolor musculoesquelético continuo, intenso y generalizado. El FMS se asocia con otros síntomas como fatiga, sueño no reparador, disfunción cognitiva/emocional y disminución de la calidad de vida relacionada con la salud. La patogenia del FMS aún no se comprende por completo, pero una cantidad cada vez mayor de evidencia respalda el vínculo entre el abuso o la negligencia emocional, física o sexual en la niñez/adultez y el desarrollo del FMS. El manejo y tratamiento de pacientes con FMS puede ser un desafío porque el síndrome es refractario a la mayoría de los tratamientos. Sin embargo, las intervenciones psicológicas, particularmente la hipnoterapia, han demostrado ser efectivas en la modulación cognitiva del dolor fibromiálgico. Los pacientes con FMS pueden beneficiarse de la hipnoterapia sola o en combinación con la terapia médica estándar. La hipnosis orientada a los síntomas tiene como objetivo reducir el dolor, la fatiga, los problemas para dormir, la ansiedad y la depresión, mientras que la hipnoterapia se enfoca en resolver conflictos emocionales y traumas no resueltos asociados con FMS. En conclusión, la hipnosis puede ser una herramienta complementaria útil y segura para controlar el dolor crónico y los síntomas disfuncionales en pacientes fibromiálgicos desafiantes.
